基于盛灿若舒郁思想从肝辨治功能性勃起功能障碍

摘　　要：勃起功能障碍是指男性阴茎不能达到或维持充分勃起以完成令人满意性交的疾病。临床调查发现,勃起功能障碍的患病人群庞大,发病趋势逐年升高,且愈发年轻化。现代医学认为,勃起功能障碍分为心因型、器质型和混合型三类。西医治疗勃起功能障碍局限性明显,不良反应较多;中医在治疗勃起功能障碍方面有着不可替代的优势,特别是针对功能性勃起功能障碍,针灸疗法效果颇佳。盛灿若教授是我国当代著名针灸学家,对男性生殖系统疾病研究颇具造诣。盛老经过数十载的临证实践与理论研究,提出肝气失调是功能性勃起功能障碍的核心病机。在总结《黄帝内经》阳气相关经典理论基础上,盛老提出舒郁思想,指出“舒”郁比“疏”郁涵盖更深,“舒”更为强调对自身肝气的柔养,柔肝与养筋紧密联系。在临床上,盛老谨守辨证论治原则,将功能性勃起功能障碍分为肝郁阳虚型、肝郁脾虚型和寒凝肝脉型3种类型。研究针对以上3种常见的功能性勃起功能障碍,介绍盛老舒肝针灸疗法,以期为临床提供参考。Erectile dysfunction(ED)refers to the inability of the penis to achieve or maintain a sustained erection to complete satisfactory sexual intercourse.Clinical surveys have found that the incidence of ED is increasing year by year,with many patients and increasingly younger.Modern medicine considers erectile dysfunction as psychological,organic and mixed.Western medicine treatment of ED has obvious limitations and many side effects.Traditional Chinese medicine is broad and profound,and has irreplaceable advantages in the treatment of ED,especially for functional ED acupuncture therapy.SHENG Canruo is a famous acupuncturist in contemporary China,with deep attainments in the study of male reproductive system diseases.After decades of clinical practice and theoretical research,he proposed that liver Qi(气)imbalance is the core pathogenesis of functional erectile dysfunction.On the basis of summarizing the classical theory of Yang(阳)Qi in Huangdi Neijing(《黄帝内经》),SHENG Canruo proposed the the thought of soothing liver Qi.He pointed out that soothing liver Qi covers more than dredging liver Qi,and soothing emphasizes the nourishing of its own liver Qi,which is closely related to nourishing the liver and tendons.In clinical practice,SHENG Canruo adheres to the principle of syndrome differentiation and treatment,and divides functional erectile dysfunction into three types:liver depression and Yang deficiency type,liver depression and spleen deficiency type,and cold coagulation liver pulse type.According to the above three common functional erectile dysfunctions,this paper introduces SHENG Canruo’s liver soothing acupuncture therapy,to provide reference for clinical practice.
