雷火灸联合帕瑞昔布钠治疗老年下肢创伤性骨折的临床研究

摘　　要：目的观察雷火灸联合帕瑞昔布钠治疗老年下肢创伤性骨折的效果。方法本研究为前瞻性研究,选择鹤壁朝阳仁爱医院2021年1月至2023年3月收治的96例老年下肢创伤性骨折患者为研究对象,随机分为常规组(48例)和试验组(48例),常规组予以常规治疗+帕瑞昔布钠治疗,试验组在常规组基础上采用雷火灸配合治疗,所有患者治疗后开展为期6个月随访,比较2组患者的症候积分,炎症细胞,下肢血液流变学,下肢疼痛、肿胀缓解情况及下肢功能恢复情况。结果在不同治疗方案下,试验组的疼痛、肿胀、瘀斑等症候积分分别为(1.9±0.5)分、(1.8±0.6)分、(1.8±0.5)分,均低于常规组[(2.3±0.8)分、(2.5±0.8)分、(2.4±0.8)分];试验组的白细胞计数(WBC)、中性粒细胞计数(NE)分别为(8.7±1.3)×10^(9)/L、(7.5±1.8)×10^(9)/L,均低于常规组[(10.3±2.26)×10^(9)/L、(8.62±1.85)×10^(9)/L];试验组的下肢全血高切黏度、下肢全血低切黏度分别为(7.35±1.3)mPa·s、(6.4±1.3)mPa·s,均低于常规组[(8.4±1.9)mPa·s、(7.5±1.9)mPa·s];试验组治疗3 d、7 d后的视觉模拟疼痛量表(VAS)评分分别为(3.66±0.27)分、(2.63±0.28)分,均低于常规组[(4.22±1.37)分、(3.53±0.85)分],下肢肿胀评分分别为(2.02±0.37)分、(1.44±0.25)分,均低于常规组[(2.66±0.83)分、(2.19±0.85)分];试验组随访第1个月、3个月、6个月时的下肢功能评分(LEFS)分别为(60±10)分、(68±10)分、(75±10)分,均高于常规组[(55±10)分、(63±10)分、(69±10)分],差异均具有统计学意义(P<0.05)。结论雷火灸联合帕瑞昔布钠治疗老年下肢创伤性骨折能有效改善患者临床症候,对减轻下肢炎症反应,促使血液流变学恢复,缓解下肢疼痛、肿胀并改善下肢运动功能均有积极意义。Objective Observation of the effect of Thunder Fire Moxibustion combined with Parexib Sodium in the treatment of traumatic fractures of the lower limbs in the elderly.Methods This study is a prospective study,selecting 96 elderly patients with traumatic fractures of the lower limbs who were admitted between January 2021 and March 2023 as the study subjects.They were randomly divided into a conventional group(48 cases)and an experimental group(48 cases)using computer randomization.The conventional group received conventional treatment and Paroxyxib sodium treatment,while the experimental group received lightning fire moxibustion combined with treatment.All patients were followed up for 6 months after treatment to compare the symptom scores of the two groups of patients,Inflammatory cells,lower limb hemorheology,relief of lower limb pain and swelling,and recovery of lower limb function.Results Under different treatment plans,the scores of pain,swelling,and ecchymosis in the experimental group were(1.9±0.5)points,(1.8±0.6)points,and(1.8±0.5)points,lower than the conventional group[(2.3±0.8)points,(2.5±0.8)points,and(2.4±0.8)points].The white blood cell(WBC)and neutrophil(NE)of the experimental group were(8.7±1.3)×10^(9)/L,(7.5±1.8)×10^(9)/L,lower than the conventional group[(10.3±2.3)]×10^(9)/L,(8.6±1.8)×10^(9)/L.The high shear viscosity of lower limb whole blood,and low shear viscosity of lower limb whole blood in the experimental group were(7.4±1.3)mPa.s,and(6.4±1.3)mPa·s,lower than the control group[(8.4±1.9)mPa·s,and(7.5±1.9)mPa.s].The visual analogue scale(VAS)scores of the experimental group after 3 and 7 days of treatment were(3.66±0.27)points and(2.63±0.28)points,lower than the conventional group[(4.22±1.37)points and(3.53±0.85)points].The lower limb swelling scores were(2.02±0.37)points and(1.44±0.25)points,lower than the conventional group[(2.66±0.83)points and(2.19±0.85)points];At the first,third,and sixth month of follow-up,the lower limb function scores(LEFS)of the experiment
