三法三穴推拿对坐骨神经损伤大鼠即刻镇痛的效果及机制

摘  要:目的观察三法三穴推拿对坐骨神经损伤大鼠即刻镇痛的效果及机制。方法用随机数字表法将27只SD大鼠分为假手术组、模型组及推拿组,每组9只。模型组、推拿组构建轻型坐骨神经慢性压迫性损伤模型模拟临床神经病理性疼痛,假手术组仅暴露神经并缝合。造模后7 d,推拿组给予三法三穴推拿干预,每穴每法1 min,共9 min;假手术组与模型组仅抓握束缚9 min,不干预。分别于造模前(T1)、造模后推拿前(T2)、推拿后(T3)采用机械缩足反射阈值(MWT)及热缩足反射潜伏期(TWL)评价即刻镇痛效果;评价结束后,取大鼠腰膨大节段脊髓背角组织,用Western blotting法检测各组大鼠脊髓背角组织脑源性神经营养因子(BDNF)、酪氨酸激酶B(TrkB)及钾氯协同转运蛋白2(KCC2)蛋白,用免疫荧光法检测各组大鼠脊髓背角组织离子化钙结合适配分子1(IBA-1)、分化簇68(CD68)、细胞癌基因Fos(c-Fos)。结果与假手术组比较,模型组T2、T3时点MWT、TWL均低(P均<0.05);与模型组比较,推拿组T3时点MWT、TWL均高(P均<0.05)。与假手术组比较,模型组脊髓背角组织BDNF、TrkB表达均高(P均<0.05),KCC2表达低(P<0.05)。与模型组比较,推拿组脊髓背角组织BDNF、TrkB表达均低(P均<0.05),KCC2表达高(P<0.05)。与假手术组比较,模型组脊髓背角组织IBA-1、CD68、c-Fos荧光强度高(P均<0.05);与模型组比较,推拿组CD68、c-Fos荧光强度低(P均>0.05)。结论三法三穴推拿在坐骨神经损伤大鼠中具有较好的即刻镇痛效果,其机制可能与改变小胶质细胞-神经元相互作用和小胶质细胞活化状态有关。Objective To observe the immediate analgesic effect and mechanism of Three-Manipulation and Three-Acupoint tuina on rats with sciatic nerve injury.Methods Twenty-seven Sprague-Dawley(SD)rats were randomly divided into the sham operation group,model group,and tuina group,with nine rats in each group.In the model and tuina groups,we established the models of mild chronic constriction injury of sciatic nerve to simulate clinical neuropathic pain,while in the sham operation group,rats only underwent nerve exposure and suturing.On the 7th day after modeling,rats in the tuina group received Three-Manipulation and Three-Acupoint tuina therapy,with each acupoint treated for 1 minute using three techniques(9 minutes in total).Rats in the sham operation and model groups were only restrained without intervention for 9 minutes.Mechanical withdrawal threshold(MWT)and thermal withdrawal latency(TWL)were measured before modeling(T1),after modeling but before tuina(T2),and after tuina(T3)to evaluate the immediate analgesic effect.After evalua⁃tion,lumbar enlargement segment spinal dorsal horn tissues were collected.Western blotting was used to detect the expres⁃sion of brain-derived neurotrophic factor(BDNF),tyrosine kinase receptor B(TrkB),and potassium-chloride cotransporter 2(KCC2)proteins,and immunofluorescence was used to detect the ionized calcium-binding adapter molecule 1(IBA-1),cluster of differentiation 68(CD68),and cellular oncogene Fos(c-Fos)expression.Results Compared with the sham op⁃eration group,the model group showed significantly lower MWT and TWL at both T2 and T3(both P<0.05).However,the tuina group had significantly higher MWT and TWL at T3 as compared with the model group(both P<0.05).The expression levels of BDNF and TrkB in the dorsal horn were higher,and KCC2 expression was lower in the model group than in the sham operation group(all P<0.05).Compared with the model group,the tuina group exhibited lower BDNF and TrkB expression but higher KCC2 expression(all P<0.05).No significant differences were
