摘　　要：目的:分析耗材集中带量采购对创伤骨科髋部骨折绿色通道手术的影响。方法:收集2022年1月至2024年2月于首都医科大学附属北京友谊医院骨科中心就诊并接受手术的髋部骨折绿色通道患者病例资料,以2023年4月实施耗材集中带量采购到位时间为分组切割点,带量采购前的患者纳入对照组(n=186),带量采购后的患者纳入研究组(n=172)。通过医院病案及信息管理系统收集患者的一般资料、围术期指标、围术期疼痛、住院期间相关费用、并发症和住院时间。结果:两组患者的性别、患侧、年龄、体质量指数、基础疾病、医疗主要支付方式等一般资料具有可比性(P>0.05)。两组患者的手术时间、麻醉方式、术前视觉模拟评分法(VAS)评分、术前住院时间差异无统计学意义(P>0.05),与对照组比,研究组的出血量、输血病例、术后清醒VAS评分、术后12 h VAS评分、内植物费用、检验检查费用、药费、治疗及其他费用、总住院费用、术后并发症、总住院时间均明显减少或降低(P<0.05)。结论:耗材集中带量采购对创伤骨科髋部骨折绿色通道手术产生积极影响,有效降低医疗费用,促进患者诊疗支出的结构优化,患者诊疗质量得到有效保障。Objective:To evaluate the impact of centralized volume-based procument of surgical consumables on green channel surgery for hip fractures in the trauma orthopedics department.Methods:The medical data of patients with hip fractures who were treated and underwent surgery through the green channel in the trauma orthopedics department of Beijing Friendship Hospital,Capital Medical University,from January 2022 to February 2024 were analyzed.Based on the implementation of centralized volume-based procument of surgical consumables in our hospital in April 2023,patients who were enrolled into hospital before procurement were collected into the control group(n=186)and patients after procurement were collected into the study group(n=172).Through the hospital medical record and information management system,the general information,perioperative indicators,perioperative pain,hospitalization related expenses,concurrent symptoms and hospitalization time of 2 groups were collected and compared.Results:There was no significant difference in general information such as gender,affected side,age,body mass index,underlying disease and main medical payment methods between the two groups(P>0.05).There was no statistically significant difference in the surgical time,anesthesia method,preoperative visual analog scale(VAS)score and preoperative hospital stay between the two groups(P>0.05).Compared with the control group,the surgical bleeding,transfusion cases,postoperative conscious VAS,postoperative 12 h VAS,implant costs,test and examination costs,medication costs,treatment costs and others,total hospital expenses,postoperative complications and total hospital stay had reduced or decreased in the study group(P<0.05).Conclusion:The centralized volume-based procument of surgical consumables has a positive impact on the green channel surgery of hip fractures in the department of trauma orthopedics,effectively reducing medical expenses,promoting the structural optimization of patient diagnosis and treatment expenditures and effectively ens
