摘  要:创伤性脊髓损伤(traumatic spinal cord injury,TSCI)一种非常常见的神经系统损伤,近年来患者数量有增加的趋势,患者的预后不良,生活质量极差。脊髓损伤(spinal cord injury,SCI)的治疗目前分为手术治疗、药物治疗、康复治疗、辅助治疗、干细胞移植,通过这几个方面回顾了既往的治疗方法,介绍了对SCI治疗的研究进展,手术重点在于早期减压,讨论了药物治疗中甲泼尼龙的使用、利鲁唑的临床试验,干细胞移植也在某些试验中表现出对患者症状的改善,针灸、高压氧治疗在临床上显现的效果,以及康复治疗对患者运动能力和膀胱功能恢复的促进。但大部分的治疗目的只能阻止损伤的继续进展,改善预后,缺乏能提升患者恢复的可能性的治疗手段。而新的治疗方法研究也在这种情况下显得尤为迫切。本文从手术、药物、康复、辅助治疗、干细胞移植等的进展情况进行综述。Traumatic spinal cord injury(TSCI)is a very common type of neurological injury.In recent years,the number of such patients has increased,and the prognosis for patients is poor and their quality of life is extremely low.Treatment of spinal cord injury(SCI)currently includes surgical treatment,drug therapy,rehabilitation therapy,adjuvant therapy,and stem cell transplantation.This article reviews previous treatment methods from these aspects and introduces research progress in SCI treatment.The focus of surgery is on the early decompression.The use of methylprednisolone in drug therapy and clinical trials of riluzole are discussed.Stem cell transplantation has shown improvement in some trials.Acupuncture and hyperbaric oxygen therapy has demonstrated some effects in clinical settings.Rehabilitation therapy can promote the recovery of patients'motor ability and bladder function.However,most of the treatment can only prevent the further progression of injury and improve the prognosis,but fails to improve the patient’s recovery.In this context,the research of new treatment methods is particularly urgent.This article reviews the progress in surgery,pharmacology,rehabilitation,adjuvant therapy and stem cell transplantation.
