摘 要:目的分析2022年至2023年沈阳市儿童EB病毒(EBV)感染的血清流行病学特征。方法收集2022年6月至2023年5月我院就诊的12083例疑似EBV感染患儿的一般资料及EBV相关血清学指标。采用LIAISION化学发光仪检测患儿血清中抗衣壳抗原IgM(VCA-IgM)抗体、抗衣壳抗原IgG(VCA-IgG)抗体、抗核抗原IgG(EBNA-IgG)抗体及抗早期抗原IgG(EA-IgG)抗体,采用实时PCR法检测EBV-DNA。比较不同性别、年龄及发病季节之间抗体阳性的差异。结果12083例患儿中VCA-IgM、VCA-IgG、EBNA-IgG、EA-IgG抗体阳性分别为1202例(9.95%)、6110例(50.57%)、5562例(46.03%)及596例(4.93%)。VCA-IgM、EA-IgG阳性率男患儿(分别为9.09%;4.44%)均低于女患儿(分别为11.10%、5.60%),差异均有统计学意义(均P<0.05)。不同年龄段(0~1岁,>1~3岁,>3~5岁,>5~10岁,>10~18岁)患儿VCA-IgM、VCA-Ig G、EBNA-IgG、EA-IgG抗体阳性率差异均有统计学意义(均P<0.05)。不同季节之间比较,VCA-IgG、EBNA-IgG和EA-IgG抗体阳性率总体差异有统计学意义(均P<0.05)。共检出14种EBV抗体阳性组合,其中,VCA-IgG、EBNA-IgG抗体均阳性患者最多,为4741例(39.24%)。进行EBV-DNA检测的3712例患儿中,EBVDNA阴性3034例(81.73%),EBV-DNA阳性678例(18.27%),EBV-DNA阴性、阳性患儿中均是VCA-IgG和EBNA-IgG抗体双阳性患者最多,分别为983例(26.48%)和194例(5.23%)。结论沈阳市儿童EBV感染以VCA-IgG、EBNA-IgG抗体双阳性为主,EBV抗体阳性率男性低于女性,不同年龄、不同发病季节患儿EBV抗体阳性率均存在差异。Objective To analyze the seroepidemiology characteristics of Epstein-Barr virus(EBV)infection in children in Shenyang.Methods From June 2022 to May 2023,serum was collected from 12083 children at Shengjing hospital of China Medical University.Serum capsid antigen(VCA)-IgM,VCA-IgG,EBV nuclear antigen-IgG(EBNA-IgG),and early antigen-IgG(EA-IgG)antibodies were detected using the LIAISION chemiluminescence immunoassay.EBV-DNA was detected using real-time PCR.Differences in antibody positivity rates between sexes,ages,and seasons of onset were compared.Results In 12083 patients,the positive rates of VCA-IgM,VCA-IgG,EBNA-IgG,and EA-IgG were 9.95%(1202/12083),50.57%(6110/12083),46.03%(5562/12083)and 4.93%(596/12083).The positive rates of VCA-IgM and EA-IgG were lower in male patients(9.09%and 4.44%,respectively)than in female patients(11.10%,5.60%;all P<0.05).The differences in the positive rates for VCA-IgM,VCA-IgG,EBNA-IgG,and EA-IgG antibodies in children of different ages were statistically significant(all P<0.05).Differences in the positive rates of VCA-IgG,EBNA-IgG,and EA-IgG antibodies were statistically significant when compared between seasons(all P<0.05).Fourteen EBV antibody-positive combinations were detected,of which the main combination was VCA-IgG and EBNA-IgG double-positive,with a total of 4741 cases(39.24%).Of the 3712 children who underwent EBV-DNA detection testing,3034(81.73%)were EBV-DNA-negative and 678(18.27%)were EBV-DNA-positive.VCA-IgG and EBNA-IgG double positivity was the most common in EBV-DNA-negative and EBV-DNA-positive children,accounting for 983(26.48%)and 194 cases(5.23%),respectively.Conclusion Both VCA-IgG and EBNA-IgG antibodies are main positive in children with EBV infection in Shenyang.The positive rate of EBV antibodies is lower in boys than in girls.The positive rates of EBV antibodies differ in children of different ages and seasons of onset.
