乳腺超声BI-RADS分类与血清癌胚抗原、糖类抗原153水平的相关性分析

摘  要:目的:分析乳腺超声报告数据系统(BI-RADS)分类与血清癌胚抗原(CEA)、糖类抗原153(CA153)水平之间的相关性,期望为乳腺癌高危人群的早期准确筛查提供依据。方法:选择我院自2021年6月至2022年6月接诊的43例进行乳腺超声检查并经病理诊断确诊为乳腺癌的女性患者作为研究对象,根据乳腺超声BI-RADS分类结果对其进行分组,其中3类组1例,4A类组13例,4B类组22例,4C类组5例,5类组2例。对所有研究对象进行乳腺超声BI-RADS分类检查和抽取静脉血进行血清CEA和CA153检测,比较各组患者血清CEA和CA153水平间的差异,收集患者临床资料并分析影响超声BI-RADS分类的因素。结果:各组患者血清CEA和CA153水平为:5类组>4C类组>4B类组>4A类组>3类组,多组间及组间两两比较分别具有统计学意义(P<0.05)。各组患者在生育、哺乳和饮酒等情况方面比较无明显统计学差异(P>0.05),但在年龄方面比较具有统计学意义(F=9.176,P=0.015)。多因素Logistic回归模型,结果发现:年龄增长、CEA和CA153是影响乳腺超声BI-RADS分类为4B、4C和5类病变的独立危险因素(P<0.05)。结论:血清CEA、CA153水平与乳腺超声BI-RADS分类间存在正相关性,是乳腺癌超声BI-RADS分类为4B、4C类和5类发生的独立危险因素,为早期乳腺癌风险筛查提供了依据。Objective:To analyze the correlation between the classification of BI-RADS and the levels of serum carcinoembryonic antigen(CEA),carbohydrate antigen 153(CA153),in order to provide a basis for early and accurate screening of high-risk groups of breast cancer.Methods:From June 2021 to June 2022,43 female patients who were diagnosed as breast cancer by breast ultrasound examination and pathological diagnosis in our hospital were selected as the research objects,which were divided into 1 case in category 3 group,13 cases in category 4A group,22 cases in category 4B group,5 cases in category 4C group and 2 cases in category 5 group according to the BI-RADS classification results of breast ultrasound.All subjects were examined by breast ultrasound BI-RADS classification and venous blood was drawn for serum CEA and CA153 detection,where the differences between the levels of serum CEA and CA153 in each group were compared and the clinical data of patients were collected and the factors affecting ultrasound BI-RADS classification were analyzed.Results:The serum CEA and CA153 levels of patients in each group were:Class 5 group>Class 4C group>Class 4B group>Class 4A group>Class 3 group,where the comparison between groups and between groups was statistically significant(P<0.05).There was no significant difference in fertility,lactation and drinking among the groups(P>0.05),while was significant difference in age(F=9.176,P=0.015).Multivariate logistic regression model showed that age increase,CEA and CA153 were independent risk factors influencing the classification of breast ultrasound BI-RADS into 4B,4C and 5 types of lesions(P<0.05).Conclusion:There is a positive correlation between serum CEA and CA153 levels and breast ultrasound BI-RADS classification,which is an independent risk factor for breast cancer classified as 4B,4C and 5 by ultrasound BI-RADS classification and provides a basis for early breast cancer risk screening.
