摘  要:本文报道1例新型冠状病毒肺炎(COVID-19)感染后游离甲状腺素(FT_(4))升高的66岁男性患者的临床诊治经过。患者甲状腺功能检测结果显示,FT_(4)明显升高,而促甲状腺激素(TSH)未受抑制。在不同检测平台复测发现FT_(4)未出现一致性升高,这提示FT_(4)的检测可能受到了干扰。放射免疫沉淀试验证明患者体内存在抗甲状腺素自身抗体(T_(4)Ab),且为IgM亚型。复习相关文献,侧重阐述甲状腺激素自身抗体干扰的识别和鉴别诊断。This article reports the clinica diagnosis and treatment of a 66-year-old male patient with elevated free thyroxine(FT_(4))following coronavirus disease 2019(COVID-19)infection.The thyroid function tests showed a significant increase in FT_(4),while thyroid stimulating hormone(TSH)was not suppressed.Re-testing on different platforms revealed inconsistent FT_(4) elevations,indicating potential interference in FT_(4) measurement.Radioimmunoprecipitation assays confirmed the presence of anti-thyroxine autoantibodies(T_(4)Ab)in the patient′s body,specifically the IgM subtype.This article reviews relevant literature,focusing on the identification and differential diagnosis of thyroid hormone autoantibodies interference.
