血清抑制素b联合性激素对男性不育症的诊断价值

摘　　要：目的探究血清抑制素b(INHB)联合性激素对男性不育症的诊断价值.方法采用回顾性分析方法,收集2021年6月-2023年12月本院接收的80例男性不育症患者病历资料(设为不育症组),根据疾病类型分为梗阻性无精子症(OA)组、非梗阻性无精子症(NOA)组,分别有43例、37例;另收集同期本院接收的明确为正常生育男性80例(设为正常组);比较正常组、不育症组患者血清INHB、性激素水平;绘制受试者工作特征曲线(ROC)评估血清INHB联合性激素对男性不育症的诊断价值.并比较OA组、NOA组血清INHB、性激素水平差异.结果不育症组血清INHB、睾酮(T)水平均低于正常组,促卵泡刺激素(FSH)、促黄体生成素(LH)水平高于正常组,差异有统计学意义(P<0.05);与OA组比较,NOA组血清INHB、T水平更低,FSH、LH水平更高,差异有统计学意义(P<0.05).绘制ROC曲线发现,血清INHB、性激素水平单独及联合诊断男性不育症的AUC均>0.750,各指标联合检测时可提高诊断价值;当各指标的cut-off值分别取118.995pg/mL、6.430IU/L、17.495nmol/L、4.595IU/L时,可获得最佳诊断价值.结论血清INHB联合性激素诊断男性不育症具有一定价值,或可鉴别诊断OA、NOA.Objective To explore the diagnostic value of serum statin b(INHB)combined with sex hormone in male infertility.Methods Retrospective analysis was performed to collect the case data of 80 male infertility patients(set as infertility group)received in our hospital from June 2021 to December 2023,and they were divided into obstructive azoospermia(OA)group and non-obstructive Azoospermia(NOA)group according to disease types,with 43 cases and 37 cases respectively.In addition,80 male patients admitted to our hospital during the same period were clearly normal fertility(set as normal group);the levels of serum INHB and sex hormone were compared between normal group and infertility group.Receiver operating characteristic curve(ROC)was drawn to evaluate the diagnostic value of serum INHB combined with sex hormones in male infertility.The levels of serum INHB and sex hormone in OA group and NOA group were compared.Results The levels of serum INHB and testosterone(T)in infertility group were lower than those in normal group,and the levels of follicle-stimulating hormone(FSH)and luteinizing hormone(LH)were higher than those in normal group,with statistical significance(P<0.05).Compared with OA group,serum INHB and T levels in NOA group were lower,and serum FSH and LH levels were higher(P<0.05).The ROC curve showed that the AUC of serum INHB and sex hormone levels in the diagnosis of male infertility were all>0.750,and the combined detection of each index could improve the diagnostic value.When the cut-off value of each index was 118.995pg/mL,6.430IU/L,17.495nmol/L and 4.595IU/L,the best diagnostic value could be obtained.Conclusion Serum INHB combined with sex hormone has a certain value in the diagnosis of male infertility,or can differentiate the diagnosis of OA and NOA.
