体外膈肌起搏器联合手指点穴及康复训练对ICU机械通气患者肺康复的疗效观察

摘　　要：目的探讨体外膈肌起搏器联合手指点穴及康复训练对ICU机械通气患者肺康复的疗效.方法将无锡市惠山区人民医院重症医学科的39例ICU机械通气获得性呼吸肌无力患者随机分为治疗组和对照组,治疗组19例,对照组20例.对照组给予常规药物治疗和机械辅助通气治疗,治疗组在对照组的基础上给予体外膈肌起搏器(EDP)联合手指点穴及康复训练治疗,连续治疗3周.治疗前后以住院相关指标、急性生理学与慢性健康状况评分系统Ⅱ(APACHEⅡ)评分、氧合指数、膈肌肌力测定为观察指标.结果治疗后,治疗组的住院时间及机械通气时间显著短于对照组(P<0.05).两组患者APACHEⅡ较治疗前相比均明显降低,且治疗组低于对照组(P<0.05).两组患者氧合指数、平静及深呼吸时膈肌移动度与治疗前相比均显著升高,且治疗组高于对照组(P<0.05).结论EDP联合手指点穴及康复训练对ICU机械通气患者肺康复疗效确切,值得临床推广.Objective To explore the therapeutic effect of extracorporeal diaphragm pacemaker combined with finger acupuncture and rehabilitation training on lung rehabilitation in ICU mechanically ventilated patients.Methods 39 ICU mechanically ventilated patients with acquired respiratory muscle weakness were randomly divided into a treatment group and a control group,with 19 cases in the treatment group and 20 cases in the control group.The control group received conventional drug treatment and mechanical assisted ventilation treatment,while the treatment group received an external diaphragm pacemaker(EDP)combined with finger acupuncture and rehabilitation training based on the control group,for a continuous treatment of 3 weeks.Before and after treatment,hospitalization-related indicators,acute physiology and chronic health status scoring systemⅡ(APACHEⅡ)score,oxygenation index,and diaphragmatic muscle strength were used as observation indicators.Results After treatment,the hospitalization time and mechanical ventilation time of the treatment group were significantly shorter than those of the control group(P<0.05).The APACHEⅡlevels in both groups of patients were significantly lower than before treatment,and the treatment group was lower than the control group(P<0.05).The oxygenation index,diaphragmatic mobility during calm and deep breathing were significantly increased in both groups of patients compared to before treatment,and the treatment group was higher than the control group(P<0.05).Conclusion The combination of EDP,finger acupuncture,and rehabilitation training has a definite therapeutic effect on lung rehabilitation in ICU mechanically ventilated patients and is worthy of clinical promotion.
