摘　　要：目的探究氢吗啡酮自控镇痛(PCA)泵治疗难治性癌痛的有效性及对心理应激的影响.方法选取我院2021年1月-2023年12月收治的201例难治性癌痛患者进行研究,按照随机数字表法分为对照组(n=100,采取盐酸吗啡缓释片治疗)与观察组(n=101,采用氢吗啡酮PCA泵治疗),观察两组镇痛有效率、焦虑抑郁情绪及应激状态.结果观察组患者镇痛有效率为96.04%,高于对照组的88.00%,P<0.05;治疗前研究对象汉密尔顿焦虑表(HAMA)及汉密尔顿抑郁表(HAMD)评分无明显差异,P>0.05,治疗后,观察组负性情绪评分低于对照组,P<0.05;治疗前研究对象去甲肾上腺素(NE)、促肾上腺皮质激素(ACTH)以及皮质醇(COR)水平无明显差异,P>0.05,治疗后观察组应激指标低于对照组,P<0.05.结论对于难治性癌痛患者采用氢吗啡酮PCA泵治疗镇痛效果更好,进而改善患者的焦虑抑郁情绪与应激指标,建议临床推广应用.Objective To investigate the effectiveness of patient-controlled analgesia(PCA)pump with hydromorphone in the treatment of refractory cancer pain and its impact on psychological stress.Methods A study was conducted on 201 patients with refractory cancer pain admitted to our hospital from January 2021 to December 2023.They were randomly divided into a control group(n=100,treated with sustained-release morphine hydrochloride tablets)and an observation group(n=101,treated with a hydromorphone PCA pump)using a random number table method.The analgesic efficacy,anxiety,depression,and stress status of the two study groups were observed.Results The effective analgesic rate of 96.04%in the observation group was higher than that of 88.00%in the control group,P<0.05;there was no difference in the Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)scores between the study subjects before treatment,P>0.05,after treatment,the observation group had a lower negative emotional score than the control group,P<0.05;there was no difference in the levels of norepinephrine(NE),adrenocorticotropic hormone(ACTH),and cortisol(COR)among the study subjects before treatment,P>0.05,after treatment,the stress indicators in the observation group were lower than those in the control group,P<0.05.Conclusion For patients with refractory cancer pain,the use of a hydromorphone PCA pump has a better analgesic effect,thereby improving their anxiety,depression,and stress indicators.It is recommended that clinical medical staff promote its application.
