三金排石汤加味对肾结石输尿管镜钬激光碎石术后患者疼痛程度及复发率的影响分析

摘　　要：目的探讨三金排石汤加味对肾结石输尿管镜钬激光碎石术(URLL)后患者疼痛程度及复发率的影响.方法选取研究对象82例,均为宜春市中医院2020年9月-2023年5月收治的肾结石患者,按随机数字表法分为两组,每组各41例.对照组行URLL治疗,观察组术后加用三金排石汤加味治疗,持续4周.比较两组临床疗效、术后恢复指标、疼痛程度、中医证候积分、炎症水平、肾功能、尿液代谢指标及复发率.结果观察组总有效率高于对照组(P<0.05);观察组腰痛减轻及拔管时间均短于对照组(P<0.05);观察组治疗后中医证候积分、VAS评分低于对照组(P<0.05);观察组治疗后IL-6、PCT水平低于对照组(P<0.05);观察组治疗后BUN、Scr低于对照组(P<0.05);观察组治疗后尿钙、尿酸低于对照组,尿镁高于对照组(P<0.05);观察组复发率低于对照组(P<0.05).结论三金排石汤加味在肾结石患者URLL术后的应用价值高,可加快结石排出,降低术后疼痛评分,加快炎症消退,并纠正尿液代谢失常,降低复发风险.Objective To explore the effect of modified Sanjin Paishi decoction on the pain degree and recurrence rate in patients with kidney stones after ureteroscopic holmium laser lithotripsy(URLL).Methods From September 2020 to May 2023,a total of 82 patients with kidney stones were admitted to Yichun Traditional Chinese Medicine Hospital were selected as the subjects.All cases were divided into two groups using random number table,with 41 cases in each group.The control group underwent URLL,and the observation group added modified Sanjin Paishi decoction after surgery for 4 weeks.The clinical efficacy,postoperative recovery indicators,pain degree,TCM syndrome score,inflammatory level,renal function,urine metabolism indicators,and recurrence rate between two groups were compared.Results Compared with the control group,the total effective rate of the observation group was higher(P<0.05);the observation group had less back pain and shorter extubation time compared to the control group(P<0.05);compared with the control group,the observation group had lower TCM syndrome score and VAS score after treatment(P<0.05);the observation group had lower levels of IL-6 and PCT compared with the control group after treatment(P<0.05);compared with the control group,the BUN and Scr of the observation group were lower after treatment(P<0.05);compared with the control group,the observation group had lower urinary calcium and uric acid and higher urinary magnesium after treatment(P<0.05);compared with the control group,the recurrence rate of the observation group was lower(P<0.05).Conclusion The modified Sanjin Paishi decoction has high application value in patients with kidney stones after URLL.It can accelerate stone excretion,reduce postoperative pain scores,accelerate inflammation resolution,correct urinary metabolic disorders,and decrease the risk of recurrence.
