摘　　要：目的探究肩关节镜辅助下TightRope钢板固定治疗肩锁关节脱位(AJD)的效果。方法回顾性抽取2021年2月至2022年3月郑州大学附属郑州中心医院收治的AJD患者37例,按照治疗方法将其分为TightRope组(19例,采用肩关节镜辅助下TightRope钢板固定治疗)和锁骨钩板组(18例,采用锁骨远端钩板内固定治疗)。比较两组围术期指标、视觉模拟评分表(VAS)评分、肩关节Constant-Murley功能评分表(CMS)评分、肩关节活动度及并发症。结果TightRope组手术时间短于锁骨钩板组(P<0.05),术中出血量少于锁骨钩板组(P<0.05)。术后6个月,TightRope组VAS评分低于锁骨钩板组,CMS评分高于锁骨钩板组,肩关节活动度大于锁骨钩板组(P<0.05)。TightRope组并发症发生率(5.26%,1/19)与锁骨钩板组(22.22%,4/18)比较,差异未见统计学意义(P>0.05)。结论肩关节镜辅助下TightRope钢板固定治疗AJD效果显著,可减少手术操作中的出血量,缓解患者疼痛症状,改善肩关节功能。Objective To investigate the effects of TightRope steel plate fixation under shoulder arthroscopy in the treatment of acromioclavicular joint dislocation(AJD).Methods A total of 37 patients with AJD admitted to Zhengzhou Central Hospital Afiliated to Zhengzhou University from February 2021 to March 2022 were retrospectively selected.According to the treatment method,the patients were divided into the TightRope group(19 cases,treated by TightRope steel plate fixation under shoulder arthroscopy)and the clavicle hook plate group(18 cases,treated by internal fixation of the distal clavicle).The perioperative indexes,visual analogue scale(VAS)score,shoulder Constant-Murley score(CMS),shoulder range of motion and incidence of complication were compared between the two groups.Results The operation duration in the TightRope group was shorter than that in the clavicle hook plate group(P<0.05),and the intraoperative blood loss in the TightRope group was less than that in the clavicle hook plate group(P<0.05).Compared with the clavicle hook plate group,the TightRope group had lower VAS score and higher CMS score 6 months after surgery,moreover,the range of motion in the shoulder joint in the TightRope group was higher than that in the clavicle hook plate group(P<0.05).There was no significant difference in the incidence of complication between the TightRope group(5.26%,1/19)and the clavicle hook group(22.22%,4/18),P>0.05.Conclusions TightRope steel plate fixation under shoulder arthroscopy has a significant effect in the treatment of AJD,which can reduce the volume of intraoperative bleeding,alleviate the pain symptoms,and improve the function of shoulder joint.
