摘　　要：目的研究外周血纤维胶凝蛋白3(Ficolin-3)、P2X7水平对儿童肺炎支原体肺炎(MPP)预后的评估价值。方法前瞻性研究。抽取2018年2月至2021年1月在汉中市人民医院进行治疗的135例MPP患儿作为观察组,另抽取同期健康体检者135例作为对照组,分析比较观察组与对照组,观察组不同严重程度、不同预后患儿的外周血Ficolin-3、P2X7水平。分析外周血Ficolin-3、P2X7水平对儿童MPP不良预后的评估效能。结果观察组Ficolin-3(t=18.04,P<0.001)、P2X7(t=30.51,P<0.001)水平高于对照组;观察组中轻度患儿的Ficolin-3(t=5.57,P<0.001)、P2X7(t=2.69,P=0.008)水平低于重度患儿,预后良好患儿的Ficolin-3(t=15.38,P<0.001)、P2X7(t=4.06,P<0.001)水平低于预后不良患儿。Ficolin-3、P2X7联合检测对MPP预后不良的诊断效能显著高于单独检测(P<0.05);通过受试者工作特征曲线分析,Ficolin-3、P2X7水平联合诊断的曲线下面积大于单一指标(P<0.05)。针对MPP预后不良患儿,Ficolin-3、P2X7的临界值分别为39.55μg/L、163.39 ng/mL。结论外周血Ficolin-3、P2X7水平对儿童MPP预后评估具有重要的意义。Objective To study the clinical value of peripheral blood ficolin-3 and P2X7 levels in the evaluation of prognosis of Mycoplasma pneumoniae pneumonia(MPP)in children.Methods:In this prospective study,135 children with MPP who were treated in Hanzhong People’s Hospital from February 2018 to January 2021 were selected as the observation group,and another 135 patients who underwent physical examination during the same period were selected as the control group.The levels of ficolin-3 and P2X7 in peripheral blood were compared between the observation group and the control group,among children with different degrees of disease severity in the observation group and among children with different prognoses in the observation group.The diagnostic efficacy of ficolin-3 combined with P2X7 in peripheral blood for poor prognosis in MPP children were analyzed.Results The levels of ficolin-3(t=18.04,P<0.001)and P2X7(t=30.51,P<0.001)in the observation group were significantly higher than those in the control group.The levels of ficolin-3(t=5.57,P<0.001)and P2X7(t=2.69,P=0.008)in children with mild MPP were significantly lower than those in children with severe MPP.The levels of ficolin-3(t=15.38,P<0.001)and P2X7(t=4.06,P<0.001)in children with good prognosis were significantly lower than those in children with poor prognosis.The diagnostic efficacy of ficolin-3 combined with P2X7 for poor prognosis in MPP children was significantly higher than those of the two single indexes(P<0.05).According to receiver operating characteristic curve analysis,the area under the curve of ficolin-3 combined with P2X7 in the diagnosis of poor prognosis in children with MPP was significantly larger than those of the two single indexes(P<0.05).For MPP children with poor prognosis,the critical values of ficolin-3 and P2X7 were 39.55μg/L and 163.39 ng/mL,respectively.Conclusions Levels of ficolin-3 and P2X7 in peripheral blood are of great significance in evaluating the prognosis of children with MPP.
