关节镜下清理术联合胫骨高位截骨术治疗中早期内翻型膝关节骨性关节炎的临床观察

摘　　要：目的探讨关节镜下清理术联合胫骨高位截骨术治疗中早期内翻型膝关节骨性关节炎的临床效果。方法回顾性抽取2021年1月至2024年1月西安交通大学第一附属医院榆林医院收治的中早期内翻型膝关节骨性关节炎患者82例,按照手术方案将其分为单纯清理组(41例,给予关节镜下清理术治疗)和截骨组(41例,给予关节镜下清理术联合胫骨高位截骨术治疗)。比较两组治疗效果、视觉模拟评分法(VAS)评分及骨关节炎指数(WOMAC)评分。结果截骨组治疗总优良率(95.12%,39/41)高于单纯清理组(80.49%,33/41),P<0.05。治疗2、4周,截骨组VAS、WOMAC评分低于单纯清理组(P<0.05)。结论中早期内翻型膝关节骨性关节炎采用关节镜下清理术联合胫骨高位截骨术治疗的效果显著,可有效缓解临床症状,促进机体恢复。Objective To investigate the clinical effect of arthroscopic debridement combined with high tibial osteotomy in the treatment of early-middle stage varus knee osteoarthritis.Methods A total of 82 patients with early-middle stage varus knee osteoarthritis admitted to Yulin Hospital,the First Affiliated Hospital of Xi'an Jiaotong University from January 2021 to January 2024 were retrospectively selected.And they were divided into the debridement alone group(41 cases,treated by arthroscopic debridement)and the osteotomy group(41 cases,treated by arthroscopic debridement combined with high tibial osteotomy).The treatment effect,visual analogue scale(VAS)score and Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)score were compared between the two groups.Results The overall excellent rate of treatment in the osteotomy group(95.12%,39/41)was higher than that in the debridement alone group(80.49%,33/41)P<0.05.After 2 and 4 weeks of treatment,the VAS and WOMAC scores of the osteotomy group were lower than those of the debridement alone(P<0.05)group.Conclusions Arthroscopic debridement combined with high tibial osteotomy is effective in the treatment of early-middle varus knee osteoarthritis,which can effectively alleviate clinical symptoms and promote recovery of the body.
