宫腔负压吸引术联合宫腔镜冷刀切除术治疗多发性子宫内膜息肉的效果

摘　　要：目的分析宫腔负压吸引术联合宫腔镜冷刀切除术(HCKR)治疗多发性子宫内膜息肉(EMP)的效果。方法队列研究。抽取2019年1月至2021年12月安阳市人民医院收治的94例多发性EMP患者,按随机数字表法分为宫腔镜下电刀切除术(HER)组和HCKR组,每组47例。HER组采用宫腔负压吸引术联合HER治疗,HCKR组采用宫腔负压吸引术联合HCKR治疗。对比两组围术期指标、子宫内膜血流参数[阻力指数(RI)、搏动指数(PI)]、术后并发症发生率、术后1年复发率和妊娠率。结果HCKR组手术时间、住院天数短于HER组,术中出血量、膨宫液吸收量少于HER组(P<0.05);术后3个月,HCKR组RI、PI低于HER组(P<0.05);HCKR组术后并发症发生率(6.38%,3/47)低于HER组(21.28%,10/47),P<0.05;HCKR组术后1年复发率(8.51%,4/47)低于HER组(27.66%,13/47),妊娠率(42.55%,20/47)高于HER组(10.64%,5/47),P<0.05。结论宫腔负压吸引术联合HCKR治疗多发性EMP效果显著,能降低出血量和膨宫液吸收量,缩短手术时间和住院天数,改善子宫内膜血流参数,且并发症发生率和复发率低,有利于促进妊娠。Objective To analyze the effect of negative pressure suction combined with hysteroscopic cold knife resection(HCKR)in the treatment of multiple endometrial polyps(EMP).Methods A total of 94 patients with multiple EMP admitted to Anyang People’s Hospital from January 2019 to December 2021 were selected for the cohort study,and they were divided into the hysteroscopic electrical resection(HER)group and the HCKR group by random number table method,with 47 cases in each group.The HER group was treated by negative pressure suction combined with HER,and the HCKR group was treated by negative pressure suction combined with HCKR.The perioperative indicators,endometrial blood flow parameters including resistance index(RI)and pulsation index(PI),incidence of postoperative complications,1-year postoperative recurrence rate and 1-year postoperative pregnancy rate were compared between the two groups.Results The operation duration and hospital stay in the HCKR group were shorter than those in the HER group,moreover,the intraoperative bleeding volume and the absorption of uterine distension fluid in the HCKR group were less than those in the HER group(P<0.05).RI and PI in the HCKR group were lower than those in the HER group 3 days after surgery(P<0.05).The incidence of postoperative complications in the HCKR group(6.38%,3/47)was lower than that in the HER group(21.28%,10/47),P<0.05.During 1 year after surgery,the recurrence rate of the HCKR group(8.51%,4/47)was lower than that of the HER group(27.66%,13/47),and the pregnancy rate of the HCKR group(42.55%,20/47)was higher than that of the HER group(10.64%,5/47),P<0.05.Conclusions Negative pressure suction combined with HCKR in the treatment of multiple EMP has significant effect.It can reduce the intraoperative bleeding volume and absorption of uterine distension fluid,shorten the operation duration and hospital stay,improve endometrial blood flow parameters,and promote pregnancy,with low incidence of complications and recurrence rate.
