摘　　要：目的探讨颈动脉超声定量参数联合血清去磷酸化未羧化基质Gla蛋白(dp-ucMGP)、闭合蛋白检测诊断急性脑梗死的价值。方法回顾性抽取2020年3月至2023年3月西安交通大学第一附属医院榆林医院收治的急性脑梗死患者86例,按照数字减影血管造影诊断的颈动脉狭窄程度,将其分为轻度组(21例)、中度组(35例)和重度组(30例)。另抽取同期体检的健康志愿者40例为对照组。比较四组颈动脉超声定量参数、dp-ucMGP、闭合蛋白水平,应用Spearman检验分析颈动脉超声定量参数、dp-ucMGP、闭合蛋白与急性脑梗死严重程度的相关性,采用受试者工作特征曲线下面积(AUC)分析颈动脉超声定量参数联合dp-ucMGP、闭合蛋白对急性脑梗死的诊断价值。结果重度组舒张末期流速(EDV)及内中膜厚度(IMT)高于轻度组、中度组、对照组(P<0.05),收缩期峰值血流速度(PSV)低于轻度组、中度组、对照组(P<0.05)。重度组dp-ucMGP及闭合蛋白水平高于轻度组、中度组、对照组(P<0.05)。Spearman分析结果显示,EDV、IMT、dp-ucMGP、闭合蛋白与急性脑梗死严重程度呈正相关(P<0.05),PSV与急性脑梗死严重程度呈负相关(P<0.05)。颈动脉超声定量参数联合dp-ucMGP、闭合蛋白诊断急性脑梗死,敏感度为93.54%,特异度为85.45%,AUC值为0.897。结论颈动脉超声定量参数、dp-ucMGP、闭合蛋白对于急性脑梗死严重程度的判断均具有重要价值,颈动脉超声定量参数联合dp-ucMGP、闭合蛋白可提升对急性脑梗死的诊断敏感度和特异性,具有较高的诊断价值。Objective To investigate the diagnostic value of carotid artery ultrasonography quantitative parameters combined with serum dephosphorylated uncarboxylated matrix Gla protein(dp-ucMGP)and occludin for acute cerebral infarction.Methods A total of 86 patients with acute cerebral infarction admitted to Yulin Hospital,the First Affiliated Hospital of Xi'an Jiaotong University from March 2020 to March 2023 were retrospectively selected,and they were divided into mild group(21 cases),moderate group(35 cases)and severe group(30 cases)according to the degree of carotid artery stenosis diagnosed by digital subtraction angiography.In addition,40 healthy volunteers who underwent physical examination during the same period were selected as the control group.The levels of carotid artery ultrasonography quantitative parameters,dp-ucMGP and occlusin were compared among the four groups.The Spearman test was used to analyze the correlations of carotid artery ultrasonography quantitative parameters,dp-ucMGP and occlusin with the severity of acute cerebral infarction.The area under the receiver operating characteristic curve(AUC)was used to analyze the diagnostic value of carotid artery ultrasonography quantitative parameters combined with dp-ucMGP and occlusin for acute cerebral infarction.Results The end-diastolic velocity(EDV)and intima-media thickness(IMT)of the severe group were higher than those of the mild group,moderate group and control group(P<0.05),while the peak systolic velocity(PSV)of the severe group was lower than that of the mild group,moderate group and control group(P<0.05).The levels of dp-ucMGP and occlusin of the severe group were higher than those of the mild group,moderate group and control group(P<0.05).The results of Spearman analysis showed that EDV,IMT,dp-ucMGP and occlusin were positively correlated with the severity of acute cerebral infarction(P<0.05),moreover,PSV was negatively correlated with the severity of acute cerebral infarction(P<0.05).The sensitivity,specificity,and AUC value of carotid arter
