摘 要:目的:探讨使用郑氏手法复位联合小夹板、钢托外固定治疗掌侧Barton骨折临床疗效。方法:自2022年5月~2023年5月期间,从本院急诊收治的掌侧Barton骨折患者中,抽取符合本次实验纳入标准的60例患者,随机分为对照组和实验组,各30例,实验组采用郑氏手法复位联合小夹板、钢托外固定制动治疗,对照组采用传统手法复位、石膏外固定治疗,术后采用常规康复锻炼,均治疗12周,统计两组临床疗效,影像学参数等。结果:通过为期12周的治疗疗程后,采用腕关节功能Cooney评分,实验组总有效率高于对照组,有统计学意义(P<0.05)。治疗12周后与治疗前比较,两组桡骨高度、尺倾角、掌倾角升高(P<0.05),组间比较,实验组优于对照组,差异有统计学意义(P<0.05)。结论:郑氏手法复位联合小夹板、钢托外固定治疗掌侧Barton骨折的临床效果满意,能有效避免桡骨高度丢失,减少腕关节畸形的发生,值得临床推广。Objective:To explore the clinical efficacy of Zheng's manual reduction combined with small splints and steel brackets for external fixation in the treatment of palmar Barton fractures.Methods:From May 2022 to May 2023,60 patients who met the inclusion criteria for this experiment were selected from the patients with Barton fracture of the palmar side admitted to the emergency department of our hospital.They were randomly divided into a control group and an experimental group,with 30 patients in each group.The experimental group received Zheng's manual reduction combined with small splints and steel plate external fixation and braking treatment,while the control group received traditional manual reduction and plaster external fixation treatment.After surgery,routine rehabilitation exercise was used,and both groups were treated for 12 weeks.The clinical efficacy and imaging parameters of the two groups were statistically analyzed.Results:After a 12 week treatment course,using the wrist joint function Cooney score,the total effective rate of the experimental group was significantly higher than that of the control group(P<0.05).After 12 weeks of treatment,compared with before treatment,the radius height,ulnar inclination angle,and palmar inclination angle of the two groups increased(P<0.05).Compared between groups,the experimental group was better than the control group,and the difference was statistically significant(P<0.05).Conclusion:The clinical effect of Zheng's manual reduction combined with small splints and steel brackets external fixation in the treatment of palmar Barton fractures is satisfactory,which can effectively avoid the loss of radial height and reduce the occurrence of wrist joint deformities,and is worthy of clinical promotion.
