改良四段法含硅胶垫圈包皮环切缝合器的应用

摘　　要：目的观察改良四段法硅胶垫圈包皮环切缝合器行包皮环切术的临床效果。方法回顾性分析2020年8月至2023年8月216例本院就诊并行包皮环切术患者的临床资料。根据使用包皮环切缝合器不同分成两组,观察组(改良四段法含硅胶垫圈包皮环切缝合器)112例,对照组(不含硅胶垫圈包皮环切缝合器)104例,比较两组的安全、有效性及手术并发症。结果两组患者包皮手术出血、迟发血肿发生率、手术后1 d、3 d和7 d包皮换药时疼痛率、术后4周缝合钉脱钉率和手工拆钉率比较,差异有统计学意义(P<0.05)。结论相对于不含硅胶垫圈一次性包皮环切缝合器,改良四段法含硅胶垫圈一次性包皮环切缝合器具有手术出血少、迟发血肿发生率低,术后包皮疼痛轻,术后4周硅胶垫圈脱钉率高和手工拆钉率低等优点,值得临床推广应用。Objective To observe the clinical effect of modified four-stage method silicone washer circumcision suture device for circumcision.Methods The clinical data of 216 patients who attended our hospital and underwent circumcision from August 2020 to August 2023 were retrospectively analysed.They were divided into two groups according to the use of different circumcision sutures,112 cases in the observation group(modified four-stage method with silicone washer circumcision suture)and 104 cases in the control group(without silicone washer circumcision suture),and the safety,effectiveness and surgical complications of the two groups were compared.Results The rates of circumcision bleeding,late-onset haematoma,pain rate during circumcision change at 1 d,3 d and 7 d after surgery,suture staple dislodgement rate at 4 weeks postoperatively and manual staple removal rate were compared between the two groups,and the differences were statistically significant(P<0.05).Conclusion Compared with the disposable circumcision suture without silicone washer,the modified four-stage method with silicone washer disposable circumcision suture has the advantages of less surgical bleeding,low incidence of delayed haematoma,mild postoperative circumcision pain,high rate of postoperative 4-week silicone washer stapling and low rate of manual stapling,which is worthy of clinical promotion and application.
