内镜下套扎术及聚桂醇硬化注射治疗Ⅱ、Ⅲ期内痔的疗效

摘　　要：目的比较内镜下套扎术与聚桂醇硬化注射治疗Ⅱ、Ⅲ期内痔的临床疗效。方法选取2021年4月至2022年3月本院收治的Ⅱ、Ⅲ期内痔患者64例,采用随机数字表法分为硬化组和套扎组,各32例,比较两组患者的有效率、住院时限、医疗费用、手术时间、术后并发症发生率及1年复发率。结果经治疗后,硬化组痊愈22例,好转7例,无效3例,有效率为90.63%;套扎组痊愈23例,好转8例,无效1例,有效率为96.88%,两组患者治疗有效率的差异无统计学意义。硬化组患者的住院时间及医疗费用均低于套扎组,差异具有统计学意义(P<0.05)。两组患者手术时间差异无统计学意义(P<0.05)。硬化组术后并发症肛门疼痛1例,便血1例,发热1例,发生率是9.38%;套扎组术后并发症肛门疼痛2例,便血2例,发生率为12.5%,两组术后并发症发生率差异无统计学意义(P>0.05)。两组术后1年复发率分别为21.88%和18.75%,差异无统计学意义。结论两种治疗方法在Ⅱ、Ⅲ期内痔中均可获得有效治疗,内镜下聚桂醇硬化注射术在住院时间及医疗费用方面更具优势。Objective To compare the clinical efficacy of endoscopic ligation and injection of polidocanol sclerotherapy for stage I,III internal hemorrhoids.Methods 64 patients with stage II and III internal hemorrhoids admitted to our hospital from April 2021 to March 2022 were selected and randomly divided into a sclerotherapy group and a ligation group,with 32 patients in each group.The effective rate,length of hospital stay,medical expenses,surgical duration,incidence of postoperative complications,and one-year recurrence rate were compared between the two groups of patients.Results After treatment,22 cases in the sclerosis group were cured,7 cases improved,and 3 cases were ineffective,with an effective rate of 90.63%.In the ligation group,23 cases were cured,8 cases improved,and 1 case was ineffective,with an effective rate of 96.88%.There was no statistically significant difference in the treatment effective rate between the two groups of patients.The length of hospital stay and medical expenses of patients in the sclerosis group were both lower than those in the ligation group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the duration of surgery between the two groups of patients(P<0.05).Postoperative complications in the sclerosis group included 1 case of anal pain,1 case of rectal bleeding,and 1 case of fever,with an incidence rate of 9.38%.There were 2 cases of postoperative anal pain and 2 cases of rectal bleeding in the ligation group,with an incidence rate of 12.5%.There was no statistically significant difference in the incidence of postoperative complications between the two groups(P>0.05).The one-year recurrence rates of the two groups after surgery were 21.88%and 18.75%,respectively,with no statistically significant difference.Conclusion Both treatment methods can effectively treat stage II and III internal hemorrhoids,with endoscopic injection of polidocanol having more advantages in terms of length of hospital stay and medical expenses.
