维持性血液透析患者心理健康状况调查及干预措施

摘　　要：目的探讨特殊封闭管理条件下长期维持性血液透析患者睡眠、情绪等心理健康状况及干预措施。方法选取封闭管理条件下长期血液透析患者66例。采用匹兹堡睡眠质量指数量表(PSQI)、症状自评量表(SCL-90)、抑郁自评量表(SDS)以及焦虑自评量表(SAS)进行心理健康状况测评,分析各量表评分与年龄、血透年限、PTH、血红蛋白、血钙、血磷、血清前蛋白等水平的相关性,及其心理、睡眠及情绪障碍的影响因素。结果依据PQSI、SAS、SDA评分标准,存在睡眠问题者为77.27%,存在焦虑情绪者为53%,抑郁情绪者为88%;特殊封闭管理场所维持性血液透析患者SCL-90总平均分水平高于全国常模(P<0.05)。结论特殊性封闭管理场所维持性血液透析患者普遍存在心理健康水平总体偏低,需予以关注。Objective To explore intervention measures for the psychological health status of long-term maintenance hemodialysis patients,including sleep and emotions,under special closed management conditions.Methods 66 long-term hemodialysis patients under closed management conditions were selected,and their mental health status was evaluated using the Pittsburgh Sleep Quality Index(PSQI),Symptom Checklist-90(SCL-90),Self Rating Depression Scale(SDS),and Self Rating Anxiety Scale(SAS).Analyze the correlation between the scale scores of selected patients and their age,duration of hemodialysis,parathyroid hormone(PTH),hemoglobin,blood calcium,blood phosphorus,serum pre protein levels,and explore the influencing factors of their psychological,sleep,and emotional disorders.Results According to the PQSI,SAS,and SDA scoring criteria,77.27%of individuals had sleep problems,53%had anxiety,and 88%had depression.The overall average SCL-90 score of maintenance hemodialysis participants in closed management facilities was higher than the national norm(P<0.05).Conclusion The overall mental health level of maintenance hemodialysis patients in special closed management places is generally low and needs attention.
