摘　　要：目的:分析双侧骶韧带悬吊术在预防经阴道全子宫切除术后盆腔脏器脱垂中的应用效果。方法:选取2018年1月—2022年12月于泗阳县康达医院接受经阴道全子宫切除术的子宫脱垂患者78例作为研究对象,随机分为对照组和观察组。对照组采用阴道全子宫切除联合阴道前后壁修补术,观察组在对照组基础上实施双侧骶骨韧带悬吊术。比较两组盆腔脏器脱垂指示点数值和盆腔脏器脱垂程度。结果:观察组阴道前壁中线-处女膜缘3 cm(Aa)、阴道后壁中线-处女膜缘3 cm(Ap)、阴道残端-Aa最远处(Ba)、阴道残端-Ap最远处(Bp)及宫颈最远端(C)数值高于对照组,后阴道长度长于对照组(P<0.001)。观察组盆腔脏器脱垂率低于对照组(P=0.038)。结论:双侧骶韧带悬吊术在预防经阴道全子宫切除术后盆腔脏器脱垂中的应用效果较好,可以有效预防患者术后盆腔脏器脱垂,值得临床应用并予以推广。Objective:To analyze the effect of bilateral sacral ligament suspension in the prevention of pelvic organ prolapse after total vaginal hysterectomy.Methods:A total of seventy-eight patients with uterine prolapse who underwent total vaginal hysterectomy in Siyang County Kangda Hospital from January 2018 to December 2022 were selected as the study objects and randomly divided into control group and observation group.The control group underwent total vaginal hysterectomy combined with anterior and posterior vaginal wall repair.The observation group underwent bilateral sacral ligament suspension on the basis of the control group.The value of pelvic organ prolapse indicator points and the degree of pelvic organ prolapse were compared between two groups.Results:In the observation group,the values of midline of the anterior wall of the vagina-margo hymen 3 cm(Aa),midline of the posterior wall of the vagina-margo hymen 3 cm(Ap),stump of the vagina-Aa furthest(Ba),stump of the vagina-Ap furthest(Bp)and cervix furthest(C)were higher than those of the control group,posterior vaginal length was longer than that of control group(P<0.001);the prolapse rate of pelvic organ in observation group was lower than that in control group(P=0.038).Conclusion:Bilateral sacral ligament suspension has a good effect on the prevention of pelvic organ prolapse after total vaginal hysterectomy,and can effectively prevent the prolapse of pelvic organs after the operation,which is worthy of clinical application and promotion.
