摘 要:目的 探讨血清视锥蛋白样蛋白-1(VILIP-1)、陷窝蛋白-1(Cav-1)在支原体肺炎并发脑炎患儿中的临床意义。方法 选取2019年10月至2022年12月南阳市中心医院收治的67例支原体肺炎并发脑炎患儿作为研究组,另外选取南阳市中心医院90例未并发脑炎的支原体肺炎患儿作为对照组。采用酶联免疫吸附法检测所有研究对象的VILIP-1、Cav-1水平;采用受试者工作特征(ROC)曲线评估VILIP-1、Cav-1对支原体肺炎并发脑炎的预测价值;采用多因素logistic回归分析探讨支原体肺炎并发脑炎的影响因素。结果 研究组患儿血清VILIP-1、Cav-1水平均高于对照组(P<0.05);VILIP-1、Cav-1预测支原体肺炎并发脑炎的曲线下面积(AUC)分别为0.761、0.853,两者联合预测的AUC为0.917。研究组患儿惊厥持续状态、脑电图异常、脑脊液MP-PCR阳性、免疫功能低下例数高于对照组患儿(P<0.05)。多因素logistic逐步回归分析显示:免疫功能低下(OR=3.442,95%CI:1.456~8.137)、惊厥持续状态(OR=3.892,95%CI:1.605~9.440)、脑电图异常(OR=4.614,95%CI:1.655~12.859)、VILIP-1≥541.84 ng·L^(-1)(OR=5.830,95%CI:1.976~17.200)、Cav-1≥33.92μg·L^(-1)(OR=6.673,95%CI:2.128~20.919)是支原体肺炎并发脑炎的影响因素(P<0.05)。结论 VILIP-1、Cav-1在支原体肺炎并发脑炎患儿中呈高表达,可预测支原体肺炎患儿并发脑炎,且二者联合检测效能更高。Objective To explore the clinical significance of serum visinin-like protein-1(VILIP-1)and caveotin-1(Cav-1)in children with Mycoplasma pneumonia complicated with encephalitis.Methods Sixty-seven children with Mycoplasma pneumonia complicated with encephalitis admitted to Nanyang Central Hospital from October 2019 to December 2022 were selected as the study group,and 90 children with Mycoplasma pneumonia without encephalitis were selected as the control group.The VILIP-1 and Cav-1 levels of all subjects were detected by enzyme-linked immunosorbent assay.The predictive value of VILIP-1 and Cav-1 in Mycoplasma pneumonia complicated with encephalitis was evaluated by receiver operating characteristic(ROC)curve.Multiple logistic regression analysis was used to investigate the influencing factors of Mycoplasma pneumonia complicated with encephalitis.Results The serum levels of VILIP-1 and Cav-1 in the study group were higher than those in the control group(P<0.05).The area under the curve(AUC)predicted by VILIP-1 and Cav-1 for Mycoplasma pneumonia complicated with encephalitis was 0.761 and 0.853,respectively,and the AUC predicted by VILIP-1 and CAV-1 combined was 0.917.The number of children in the study group with persistent convulsion,abnormal EEG,MP-PCR positive cerebrospinal fluid and immunosuppression was higher than that in the control group(P<0.05).Immunosuppression(OR=3.442,95%CI:1.456-8.137),persistent eclampsia(OR=3.892,95%CI:1.605-9.440),abnormal EEG(OR=4.614,95%CI:1.655-12.859),VILIP-1≥541.84 ng·L^(-1)(OR=5.830,95%CI:1.976-17.200),Cav-1≥33.92μg·L^(-1)(OR=6.673,95%CI:2.128-20.919)were the influencing factors of Mycoplasma pneumonia complicated with encephalitis(P<0.05).Conclusion VILIP-1 and Cav-1 are highly expressed in children with Mycoplasma pneumonia complicated with encephalitis,which can predict the complicated encephalitis in children with Mycoplasma pneumonia,and their combined detection efficiency is higher.
