祛腐生肌中药直肠滴入对肛瘘术后患者临床效果的影响

摘　　要：目的 探讨祛腐生肌中药直肠滴入对肛瘘术后患者临床效果的影响。方法 选取2022年2月至2023年1月于南阳市中心医院接受手术治疗的肛瘘患者110例,随机分为常规组和直肠滴入组,各55例,常规组术后采用高锰酸钾溶液坐浴和龙珠软膏治疗,直肠滴入组在常规基础上联合祛腐生肌中药直肠滴入治疗。比较两组创面修复疗效、康复进程、术后不同时间点疼痛程度[视觉模拟量表(VAS)评分]、创面愈合情况、血管内皮相关指标[血管内皮生长因子(VEGF)、血管内皮生长因子受体2(VEGFR-2)]水平、炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)]水平、术后感染发生情况。结果 两组患者创面修复总有效率均为100.00%,其中直肠滴入组患者创面修复显效率高于常规组(P<0.05);直肠滴入组疼痛消失、水肿消退、创面渗出物消失及创面愈合时间均短于常规组(P<0.05);术后3、7、14 d,直肠滴入组VAS评分均低于常规组(P<0.05);术后14 d,创面愈合情况优于常规组(P<0.05);术后7 d,直肠滴入组血清VEGF、VEGFR-2水平均高于常规组,IL-6、TNF-α、IL-8水平均低于常规组(P<0.05);直肠滴入组术后感染发生率与常规组比较,差异无统计学意义(P>0.05)。结论 祛腐生肌中药直肠滴入治疗肛瘘术后患者,可减轻炎症反应,提升VEGF、VEGFR-2表达,促进创面愈合,减轻术后疼痛,加快康复进程。Objective To investigate the effects of Qufu Shengji traditional Chinese medicine rectal drip on clinical efficacy of postoperative patients with anal fistula.Methods A total of 110 patients with anal fistula who received surgical treatment in Nanyang Central Hospital from February 2022 to January 2023 were selected and randomly divided into conventional group and rectal drip group,55 cases in each group.The conventional group was treated with potassium permanganate solution sat bath and dragon bead ointment after surgery,the rectal drip group was treated with Qufu Shengji traditional Chinese medicine on the basis of conventional treatment.The effect of wound repair,rehabilitation process,pain degree[visual analog scale(VAS)score]at different time points after surgery,wound healing,vascular endothelial related indexes[vascular endothlial growth factor(VEGF),vascular endothelial growth factor receptor 2(VEGFR-2)],inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8)],and postoperative infection were compared between the two groups.Results The total effective rate of wound repair in both groups was 100.00%,the effective rate of wound repair in rectal drop group was higher than that in conventional group(P<0.05).The pain disappeared,edema subsided,wound exudate disappeared and wound healing time in rectal drop group were shorter than those in conventional group(P<0.05).After 3,7 and 14 days of surgery,the VAS scores in the rectal drip group were lower than those in the conventional group(P<0.05).After 14 days of surgery,the wound healing was better than that in the conventional group(P<0.05).After 7 days of surgery,serum VEGF and VEGFR-2 levels in rectal drop group were higher than those in conventional group,and serum IL-6,TNF-αand IL-8 levels were lower than those in conventional group(P<0.05).The incidence of postoperative infection had no difference between rectal drip group and conventional group(P>0.05).Conclusion Qufu Shengji traditional Chinese medicine rectal drip ca
