摘 要:目的 研究自适应统计迭代重建(ASIR)算法对肝硬化患者碘摄入量、辐射剂量及门静脉计算机断层扫描(CT)图像质量的影响。方法 以2020年1月至2022年12月河南中医药大学第一附属医院收治的68例肝硬化患者为研究对象,采用随机数字表法分为滤波反投影法(FBP)组(34例)和ASIR组(34例)。所有患者均进行门静脉CT检查,其中FBP组进行FBP重建(0%ASIR),对比剂用量450 mgI·kg^(-1),管电压100 kV;ASIR组进行ASIR重建(40%ASIR),对比剂用量350 mgI·kg^(-1),管电压100 kV。统计两组一般资料,比较两组碘摄入量、辐射剂量及门静脉CT、对比噪声比(CNR)、信噪比(SNR)值、图像质量主观评分。结果 ASIR组碘摄入量低于FBP组(P<0.05);ASIR组和FBP组有效剂量(ED)、CT剂量容积指数(CTDIvol)、剂量长度乘积(DLP)比较,差异无统计学意义(P>0.05)。ASIR组门静脉主干、右支、左支CT、CNR、SNR值及门静脉图像噪声、伪影、门静脉3级分支结构显示主观评分均高于FBP组(P<0.05)。结论 与FBP重建相比,ASIR重建用于肝硬化患者门静脉CT检查可提高门静脉CT、CNR、SNR值,提高CT成像的客观和主观图像质量,但可有效降低碘摄入量,且不会增加辐射剂量,有助于提高安全性。Objective To study the effect of adaptive statistical iterative reconstruction(ASIR)algorithm on iodine intake,radiation dose and portal vein computed tomography(CT)image quality in patients with cirrhosis.Methods A total of 68 patients with cirrhosis admitted to the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from January 2020 to December 2022 were divided into the filtered back projection(FBP)group(34 cases)and the ASIR group(34 cases)by random number table method.All patients underwent portal vein CT examination,in which the FBP group underwent filtered back projection(FBP)reconstruction(0%ASIR)with contrast agent dosage of 450 mgI·kg^(-1) and tube voltage of 100 kV.ASIR reconstruction was performed in the ASIR group(40%ASIR)with contrast agent dosage of 350 mgI·kg^(-1) and tube voltage of 100 kV.The general data of the two groups were analyzed,and the iodine intake,radiation dose,portal vein CT,contrast-to-noise ratio(CNR),signal-to-noise ratio(SNR)values and subjective score of image quality were compared between the two groups.Results Iodine intake in the ASIR group was lower than that in the FBP group(P<0.05).There was no significant difference in iodine intake,effective dose(ED),CT dose volume index(CTDIvol)and dose length product(DLP)between the ASIR group and the FBP group(P>0.05).CT,CNR,SNR values of the main,right and left branches of the portal vein in the ASIR group,as well as portal vein image noise,artifact and score of level 3 branch structure of the portal vein showed that the subjective was higher than that of the FBP group(P<0.05).Conclusion Compared with FBP reconstruction,ASIR reconstruction for portal vein CT examination in patients with cirrhosis could increase the portal vein CT,CNR and SNR values,and improve the objective and subjective image quality of CT imaging,but could effectively reduce iodine intake without increasing radiation dose,which was conducive to improving safety.
