温肾泄浊汤联合前列地尔治疗糖尿病肾病的临床效果

摘　　要：目的:分析温肾泄浊汤联合前列地尔治疗糖尿病肾病的临床效果。方法:将2021年1月—2023年4月睢宁县中医院收治的糖尿病肾病患者68例,随机分为对照组(n=34,前列地尔治疗)和观察组(n=34,温肾泄浊汤联合前列地尔治疗)。对比两组治疗效果,肾功能指标、中医证候积分和炎症因子水平。结果:观察组治疗总有效率高于对照组(P<0.05)。治疗前,两组尿素氮(BUN)、血肌酐(Scr)、血尿酸(UA)水平及内生肌酐清除率(Ccr)比较,无统计学差异(P>0.05);治疗后,两组BUN、Scr、UA水平下降,Ccr升高,且观察组改善幅度大于对照组(P<0.05)。治疗前,两组口干舌燥、倦怠乏力、腰背刺痛、小便浑浊积分比较,无统计学差异(P>0.05);治疗后,两组口干舌燥、倦怠乏力、腰背刺痛、小便浑浊积分下降,且观察组低于对照组(P<0.05)。治疗前,两组白细胞介素-10(IL-10)、白细胞介素-8(IL-8)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平比较,无统计学差异(P>0.05);治疗后,两组IL-10、IL-8、IL-6、TNF-α水平下降,且观察组低于对照组(P<0.05)。结论:温肾泄浊汤联合前列地尔治疗糖尿病肾病的临床效果较好,可改善患者肾功能,减轻临床症状和炎性反应。Objective:To analyze clinical effect of Wenshen Xiezhuo Decoction combined with alprostadil in treatment of diabetic nephropathy.Methods:From January 2021 to April 2023,68 patients with diabetic nephropathy admitted to Suining Traditional Chinese Medicine Hospital were randomly divided into control group(n=34,treated with alprostadil)and observation group(n=34,treated with Wenshen Xiezhuo Decoction and alprostadil).The treatment effects,renal function indicators,TCM syndrome scores,and levels of inflammatory factors were compared.Results:In observation group,total effective rate of treatment was higher than control group(P<0.05).Before treatment,there was no significant difference in the levels of urea nitrogen(BUN),serum creatinine(Scr),serum uric acid(UA)and endogenous creatinine clearance rate(Ccr)between the two groups(P>0.05).After treatment,the levels of BUN,Scr and UA decreased and Ccr increased in the two groups,and the improvement of observation group was greater than control group(P<0.05).Before treatment,there was no significant difference in the scores of dry mouth and tongue,fatigue and acratia,stabbing pain in lower back and back,turbid urine between the two groups(P>0.05).After treatment,the scores of dry mouth and tongue,fatigue and acratia,stabbing pain in lower back and back,turbid urine decreased in the two groups,and the scores of observation group were lower than control group(P<0.05).Before treatment,there was no significant difference in the levels of interleukin-10(IL-10),interleukin-8(IL-8),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)between the two groups(P>0.05).After treatment,the levels of IL-10,IL-8,IL-6 and TNF-αdecreased in the two groups,and the levels of observation group were lower than control group(P<0.05).Conclusion:The clinical effect of Wenshen Xiezhuo Decoction combined withalprostadil in the treatment of diabetic nephropathy is good,which can improve the renal function of patients and relieve clinical symptoms and inflammatory reactions.
