古方治疗淋巴瘤用药规律探析

摘　　要：目的利用数据挖掘分析中医古方治疗淋巴瘤用药规律,结合现代相关文献,探讨古今用药变化及对现代临床的启示意义。方法收集并整理中医古籍中方名或主治包含“瘰疬”“痰核”的古方。采用Excel2019构建数据库,应用SPSS Statistics 25.0统计用药频率,并进行聚类分析,利用Python3.10进行关联规则分析,利用Gephi0.9.2进行复杂网络分析。结果共纳入259首古方,包含483味中药,用药频次为2256次,其中使用频次≥30的药物有12味,核心药物包括甘草、当归、乳香、没药、连翘等。关联规则分析显示,置信度排名前4的药物组合为乳香-没药、乳香-当归、没药-当归、当归-甘草。聚类分析得到5种功用不同的药物组合,提示古方治疗淋巴瘤以补益正气贯穿全程,兼用辛温通络、清热化痰、和营养血、祛风通玄、疏肝健脾等治法。结论淋巴瘤古今用药基本一致,古方重视应用解表药,现代临床应用化痰药比重增加,高频药物当归在淋巴瘤治疗中有特殊意义。Objective To analyze the medication law of ancient prescriptions in the treatment of lymphoma using data mining;To explore the changes of ancient and modern medication and the enlightenment to modern clinic by combining modern relevant literature.Methods Ancient prescriptions with names or indications containing“scrofula”and“phlegm kernel”were collected and organized.Excel 2019 was used to build a database.SPSS Statistics 25.0 was used to analyze the application frequency,and clustering analysis and complex network analysis were carried out.Python 3.10 was used for association rule analysis and Gephi0.9.2 was used for complex network analysis.Results A total of 259 ancient prescriptions were included,including 483 kinds of Chinese materia medica,with a total frequency of 2256 times,among which 12 kinds of Chinese materia medica were used more than 30 times.The core medicines included Glycyrrhizae Radix et Rhizoma,Angelicae Sinensis Radix,Olibanum,Myrrha,Forsythiae Fructus,etc.Association rule analysis showed that the top four drug combinations with confidence were Olibanum-Myrrha,Olibanum-Angelicae Sinensis Radix,Myrrha-Angelicae Sinensis Radix,Angelicae Sinensis Radix-Glycyrrhizae Radix et Rhizoma.Five different functional drug combinations were obtained by clustering analysis,suggesting that the ancient prescriptions used in the treatment of lymphoma through the whole process of tonifying and invigorating healthy qi,with warming and dredging collaterals,clearing heat and reducing phlegm,reconcile nutritive level and nourishing blood,dispelling wind and clear Xuanfu,soothing liver and strengthening spleen and other treatment methods.Conclusion The ancient and modern drugs used in lymphoma are basically consistent.The ancient prescriptions attached great importance to the application of antisuperficial drugs.The proportion of phlegm reducing drugs in modern clinical application increases,and high frequency drugs such as Angelicae Sinensis Radix have special significance in the treatment of lymphoma.
