基于R语言数据挖掘探讨中医药治疗寒湿痹阻型膝骨关节炎用药规律

摘　　要：目的基于R语言利用数据挖掘技术探讨中医药治疗寒湿痹阻型膝骨关节炎(KOA)的用药规律,为寒湿痹阻型KOA的中医药治疗提供参考依据。方法检索中国知识资源总库(CNKI)、中文科技期刊数据库(VIP)、中国学术期刊数据库(万方数据)、中国生物医学文献服务系统(SinoMed)中医药治疗寒湿痹阻型KOA研究文献,将处方录入Excel2010,构建方剂数据库,利用R4.3.1软件进行频数统计、关联规则分析、聚类分析。结果共纳入处方52首,共包含中药102味,用药频次前10位药物分别是牛膝、独活、桂枝、当归、白芍、黄芪、秦艽、羌活、附子、防风;药性以温、平为主,药味以苦、辛、甘为主;归经以肝经、肾经、脾经为主。关联规则分析获得以黄芪、羌活、独活、海风藤、桂枝、川芎、桑枝为核心的药物组合;层次聚类分析得到4组药物组合。结论中医药治疗寒湿痹阻型KOA多以除湿止痛、温经通络为主,兼以补益肝肾,核心药物组合由蠲痹汤、芪附麻辛桂姜汤加减化裁而成。Objective To explore medication law of TCM in treating knee osteoarthritis(KOA)with cold and dampness obstruction type based on R language;To provide a reference for the treatment of KOA with cold and dampness obstruction type.Methods The research literature about TCM for the treatment of KOA with cold and dampness obstruction type was retrieved from CNKI,VIP,Wanfang Data,and SinoMed.The prescriptions were entered into Excel 2010 to build prescription database.R4.3.1 software was used for frequency statistics,association rule analysis,and clustering analysis.Results Totally 52 prescriptions were included,involving 102 kinds of Chinese materia medica.The top ten high-frequency medicines Cyathulae Radix,Angelicae Pubescentis Radix,Cinnamomi Ramulus,Angelicae Sinensis Radix,Paeoniae Radix alba,Astragali Radix,Gentianae Macrophyllae Radix,Notopterygii Rhizoma et Radix,Aconiti Lateralis Radix Praeparata,Saposheikovize Radix.The main tastes were bitter,pungent and sweet;the main meridians were liver,lung,and spleen meridians.Association rule analysis obtained drug combinations with Astragali Radix,Notopterygii Rhizoma et Radix,Angelicae Pubescentis Radix,Piperis Kadsurae Caulis,Cinnamomi Ramulus,Chuanxiaong Rhizoma,Mori Ramulus as the core;four drug combinations were obtained through hierarchical clustering analysis.Conclusion TCM treatment for KOA with cold and dampness obstruction type mainly takes dehumidification and pain relief,warming meridian and dredging collaterals,and supplements the liver and kidney.The core drug combination can obtain Jubi Decoction and modified Qifu Maxin Guijiang Decoction.
