摘 要:目的:分析不同透析频率与方式对慢性肾功能衰竭尿毒症患者的疗效。方法:选取南通市第三人民医院2022年2月—2023年2月收治的慢性肾功能衰竭尿毒症患者89例为研究对象,以随机数字表法分为对照组(n=43)和观察组(n=46)。对照组应用常规血液透析治疗,观察组在对照组基础上增加血液透析治疗次数并联合血液透析滤过治疗。对比两组小分子毒素及大分子毒素水平、炎性指标、营养指标、并发症发生率、死亡率。结果:干预后,两组血肌酐、血尿素氮、血尿酸水平均低于干预前,且观察组低于对照组(P<0.05);干预后,观察组β2-微球蛋白水平低于对照组(P<0.001);干预后,两组C反应蛋白、血红蛋白水平高于干预前,观察组C反应蛋白水平低于对照组,血红蛋白、白蛋白水平高于对照组(P<0.05);观察组并发症发生率低于对照组(P=0.033);观察组死亡率低于对照组(P=0.020),对照组死亡风险高于观察组(P=0.033)。结论:在血液透析基础上结合血液滤过治疗,并增加透析频率,能够提高对大分子毒素和小分子毒素的清除率,减轻炎症水平,改善患者的营养状况,安全性好。Objective:To analyze the effect of different dialysis frequency and mode on uremia patients with chronic renal failure.Methods:Eighty-nine patients with chronic renal failure uremia admitted to Nantong Third People's Hospital from February 2022 to February 2023 were selected as the study objects,and were divided into control group(n=43)and observation group(n=46)by random number table method.The control group received routine hemodialysis treatment,and the observation group increased the frequency of hemodialysis treatment and combined hemodialysis filtration treatment on the basis of the control group.The levels of small molecular toxins and large molecular toxins,inflammatory indexes,nutritional indexes,incidence of complications and mortality were compared between the two groups.Results:After intervention,serum creatinine,blood urea nitrogen and blood uric acid levels in the two groups were lower than before intervention,and observation group was lower than control group(P<0.05);after intervention,the level ofβ2-microglobulin in observation group was lower than that in control group(P<0.001);after intervention,the levels of C-reactive protein and hemoglobin in two groups were higher than before intervention,but the levels of C-reactive protein in observation group were lower than control group,and the levels of hemoglobin and albumin were higher than control group(P<0.05);the complication rate of observation group was lower than that of control group(P=0.033);the mortality rate in the observation group was lower than that in the control group(P=0.020),and the risk of death in the control group was higher than that in the observation group(P=0.033).Conclusion:Combined with hemofiltration therapy on the basis of hemodialysis and increasing the frequency of dialysis,it can improve the clearance rate of macromolecular toxins and small molecular toxins,reduce the level of inflammation,improve the nutritional status of patients,and have good safety.
