摘　　要：目的:探讨间歇性经口至食管管饲法(IOE)在脑卒中吞咽障碍患者中的应用效果。方法:选择2022年10月—2023年9月重庆三峡医药高等专科学校附属人民医院收治的脑卒中吞咽障碍患者60例,随机分为观察组与对照组,每组30例。对照组采用常规胃管鼻饲护理,观察组采用IOE护理。对比两组一次性插管成功率、耐受率、舒适度及并发症发生率。结果:观察组一次性插管成功率、患者可耐受率高于对照组,差异有统计学意义(P<0.05)。第1天,两组咽喉疼痛、吞咽不适评分比较,差异无统计学意义(P>0.05);第7天,两组咽喉疼痛、吞咽不适评分降低,观察组低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P=0.038)。结论:IOE可提高脑卒中吞咽功能障碍患者一次性插管成功率、耐受率及舒适度,降低并发症发生率。Objective:To investigate the effect of intermittent oro-esophageal tube feeding(IOE)in stroke patients with dysphagia.Methods:Sixty stroke patients with dysphagia admitted to the Affiliated People's Hospital of Chongqing Three Gorges Medical College from October 2022 to September 2023 were randomly divided into observation group and control group,with thirty cases in each group.The control group received conventional gastric tube feeding,and the observation group received IOE nursing.The success rate,tolerance rate,comfort and complication rate of one-time intubation were compared between the two groups.Results:The success rate and patient tolerance rate of one-time intubation in observation group were higher than those in control group,and the difference was statistically significant(P<0.05).On day 1,there was no significant difference in the scores of throat pain and swallowing discomfort between the two groups(P>0.05);on the 7th day,the scores of throat pain and swallowing discomfort in both groups were decreased,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).The complication rate of the observation group was lower than that of the control group,and the difference was statistically significant(P=0.038).Conclusion:IOE can improve the success rate,tolerance rate and comfort of one-time intubation in stroke patients with swallowing dysfunction,and reduce the incidence of complications.
