摘　　要：目的:检验自评8条目痴呆筛查问卷(AD8)在社区老年人群痴呆筛查中的信效度,探究不同受教育程度的截断值。方法:在成都市温江区某社区505个样本中采用项目分析、验证性因子分析方法探索自评AD8的结构效度,以简易智能精神状态量表(MMSE)为校标检验关联效度,使用Cronbachα系数和Spear Brown系数检验信度。通过绘制受试者工作曲线(ROC)分析不同教育程度人群中的自评AD8截断值。结果:自评AD8的内部一致性信度为0.761,折半信度为0.822。验证性因子分析结果显示,AD8的模型拟合度较好(χ^(2)/ν=3.247,RMSEA=0.067,GFI=0.969,NFI=0.916,IFI=0.941)。不同受教育水平人群(文盲与非文盲)自评AD8的痴呆筛查截断值均为2,在文盲人群中的敏感度为0.715,特异度为0.882,曲线下面积(AUC)为0.808,高于非文盲人群(敏感度=0.481,特异度=0.770,AUC=0.644)。结论:自评AD8作为社区老年人痴呆筛查的工具具有较好的信度和效度,但其在文盲群体中的筛查效果优于非文盲群体。Objective:To test the reliability and validity of self-rating AD8 in the screening of dementia in the community elderly population,and to explore the cut-off value of different education levels.Methods:Item analysis and confirmatory factor analysis were employed to scrutinize the structural validity of self-rating AD8 in a sample of 505 individuals from a community in Wenjiang district,Chengdu.The Mini-mental State Examination(MMSE)was utilized as the criterion measure for testing convergent validity,while Cronbach'sαcoefficient and Spearman-Brown coefficient were applied to assess reliability.Receiver operating characteristic curve(ROC)analysis was conducted to analyse the optimal cut-off values for self-rating AD8 amongpopulation with different levels of education.Results:The internal consistent reliability of self-rating AD8 was 0.761,and the split-half reliability was 0.822.The results of confirmatory factor analysis show that the fitting degree of the model of AD8 was good(χ^(2)/ν=3.247,RMSEA=0.067,GFI=0.969,NFI=0.916,IFI=0.941).The cutoff value of self-rated AD8 for people with different education levels(illiterate and non-illiterate)was 2,and the sensitivity,specificity and area under the receiver operating characteristic curve(AUC)of illiterate people were 0.715,0.882 and 0.808 respectively,which was higher than that of non-illiterate people(sensitivity=0.481,specificity=0.770,AUC=0.644).Conclusion:Self-rating AD8 has good reliability and validity as a tool for dementia screening of the elderly in the community,but its screening effect in illiterate groups is better than that in non-illiterate groups.
