冲击波联合超声引导PRP注射治疗膝骨关节炎的临床研究

摘　　要：目的:探究冲击波联合超声引导富血小板血浆(PRP)注射治疗膝骨关节炎的临床效果。方法:选取2023年2月—2024年1月建湖县人民医院收治的100例膝骨关节炎患者作为研究对象,按照随机数表法分为对照组与观察组,每组各50例。对照组应用超声引导PRP注射治疗,观察组应用冲击波联合超声引导PRP注射治疗。比较两组临床疗效、膝关节功能评分、肌力水平分级情况、疼痛程度和膝关节活动情况。结果:两组治疗总有效率比较,观察组优于对照组,差异有统计学意义(P<0.05)。治疗前,两组美国西部安大略和麦克马斯特大学骨关节指数(WOMAC评分)评分比较,差异无统计学意义(P>0.05);治疗后,两组WOMAC评分较治疗前明显降低,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组视觉模拟尺度评分法(VAS评分)、膝关节主动活动度(ROM)得分比较,差异无统计学意义(P>0.05);治疗后,观察组VAS评分低于对照组,而观察组ROM得分高于对照组,差异有统计学意义(P<0.05)。两组肌力水平分级情况比较,观察组4级与5级的病例数均高于对照组,差异有统计学意义(P<0.05)。结论:应用冲击波联合超声引导PRP注射治疗膝骨关节炎能获得更好的效果,患者的疼痛明显减轻,主动活动度、肌力水平均得到大幅度提升,有效促进了膝关节功能的康复。Objective:To explore the clinical effect of shock wave combined with ultrasound-guided platelet rich plasma(PRP)injection in the treatment of knee osteoarthritis.Method:100 patients with knee osteoarthritis admitted to Jianhu County People's Hospital from February 2023 to January 2024 were selected as research objects and divided into control group and observation group according to random number table method,with 50 cases in each group.The control group was treated with ultrasound guided PRP injection,and the observation group was treated with shock wave combined with ultrasound guided PRP injection.Clinical efficacy,knee function score,muscle strength level grading,pain degree and knee activity of the two groups were compared.Result:Compared with the total effective rate of the two groups,the observation group was superior to the control group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in WOMAC scores between the two groups(P>0.05).After treatment,the WOMAC score of the two groups was significantly lower than that before treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in visual analogue scale(VAS)score and active knee motion(ROM)score between the two groups(P>0.05).After treatment,VAS score of observation group was lower than control group,while ROM score of observation group was higher than control group,the difference was statistically significant(P<0.05).Compared with the grading of muscle strength level between the two groups,the number of cases in grade 4 and grade 5 in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The application of shock wave combined with ultrasound guided PRP injection in the treatment of knee osteoarthritis can obtain better results,the pain of patients is significantly reduced,and the level of active motion and m
