关节镜下不同缝合法在膝关节外侧盘状半月板损伤患者治疗中的对比观察

摘　　要：目的:观察比较关节镜下采用不同缝合法治疗膝关节外侧盘状半月板损伤的临床效果。方法:选取2020年6月—2023年6月龙岩市第一医院治疗的84例膝关节外侧盘状半月板损伤接受关节镜手术治疗患者作为研究对象,采用方便抽样法随机分为全内组和内外组。每组各42例。全内组术中采取全内缝合法,内外组术中采取内外缝合法。观察并比较两组患者的手术情况、术后体征情况、视觉模拟量表(VAS)评分、Lysholm评分及术后并发症情况。结果:全内组术中出血量、手术时间、术后住院时间均显著低于内外组,差异有统计学意义(P<0.05);关节间隙压痛、关节绞索、Mc Murray征阳性率均显著低于内外组,差异有统计学意义(P<0.05);术后1个月、3个月,两组关节VAS评分比术前显著降低,Lysholm评分比术前显著升高,且全内组显著优于内外组,差异有统计学意义(P<0.05);两组均无术后并发症,仅内外组在术后3个月出现膝关节屈伸活动伴有不适、膝关节过紧情况,发生率显著高于全内组,差异有统计学意义(P<0.05)。结论:膝关节外侧盘状半月板损伤患者关节镜术中采取全内缝合法相较于内外缝合法可以有效缩短手术时间,降低术中出血量,减轻关节疼痛,避免神经、血管损伤从而促进膝关节功能恢复,避免术后出现膝关节屈伸活动不适、关节过紧情况。Objective:To observe and compare the clinical effects of different arthroscopic suture methods in the treatment of discoid lateral meniscus injury.Method:84 patients with discoid lateral meniscus injury who underwent arthroscopic surgery in the hospital from June 2020 to June 2023 were selected as the study oubjects.They were randomly divided into the all-inside group and the inside-outside group,with 42 cases in each group.The patients of all inside group were treated with tatal internal suture.while the patients of inside-outside group were treated with inside-outside suture.Surgical conditions,postoperative signs,visual analogue scale(VAS)scores,Lysholm scores,and postoperative complications were compared between the two groups.Result:Intraoperative blood loss,surgical time and postoperative hospital stay of the all-inside group were significantly less than those of the inside-outside group(P<0.05).The positive rates of joint space tenderness,joint locking,and Mc Murray sign in the all-inside group were significantly lower than those in the inside-outside group(P<0.05).1 month and 3 months after surgery,VAS scores of the two groups were significantly lower than those before surgery,and Lysholm scores were significantly higher than those before surgery.Above scores of the all-inside group were significantly better than those of the inside-outside group(P<0.05).No significant postoperative complications were observed in either group,but knee flexion and extension with discomfort and knee tightness in the appeared 3 months after surgery.The incidence was significantly higher than that in the all-inside group(P<0.05).Conclusion:Compared with inside-outside suture,applying allinside suture in arthroscopic surgery for patients with discoid lateral meniscus injury can effectively shorten the surgical time,reduce intraoperative blood loss,alleviate joint pain,and avoid nerve and vascular damage,thus promoting knee function recovery and avoiding postoperative discomfort in knee flexion and extension as well as joint ti
