后路短节段+伤椎单侧置钉对侧经椎弓根植骨治疗对胸腰椎骨折患者的效果

摘　　要：目的:观察并分析对胸腰椎骨折患者应用后路短节段+伤椎单侧置钉对侧经椎弓根植骨治疗的效果。方法:选取2020年3月—2023年6月贵航贵阳医院收治的106例胸腰椎骨折患者作为研究对象,随机分为对照组和观察组,每组各53例。对照组采用后路长节段跨伤椎8钉内固定+椎板横突间植骨治疗,观察组采用后路短节段+伤椎单侧置钉对侧经椎弓根植骨治疗。对比两组围手术期指标、伤椎影像学参数变化、症状改善情况。结果:观察组手术总时间与住院总时间均短于对照组,术中总出血量少于对照组,差异有统计学意义(P<0.05);治疗后,观察组伤椎前缘高度比高于对照组,后凸Cobb角低于对照组,差异有统计学意义(P<0.05);治疗后,观察组腰椎功能障碍指数评分量表(ODI)评分低于对照组,日本骨科协会(JOA)评分高于对照组,差异有统计学意义(P<0.05)。结论:对胸腰椎骨折患者应用后路短节段+伤椎单侧置钉对侧经椎弓根植骨治疗,能够改善围术期指标,缓解临床症状,优化伤椎影像学参数,可行性良好。Objective:To observe and analyze the effect of posterior short segment combined with unilateral fixation of injured vertebra on the changes of imaging parameters of injured vertebra.Method:A total of 106 patients with thoracolumbar fracture admitted to the spinal Department of Guihang Guiyang Hospital from March 2020 to June 2023 were selected as the study objects and randomly divided into control group(n=53)and observation group(n=53).The patients of control group were treated with 8-nail internal fixation across the injured vertebra in the long posterior segment plus interlaminae transversoid bone grafting,and The patients of observation group were treated with unilateral transvertebral arch bone grafting in the short posterior segment plus the injured vertebra.The perioperative indexes, the changes in imaging parameters of the injured vertebra and the improvement of symptoms were compared between the two groups. Result: The total operation time and hospitalization time in the observation group were shorter than those in the control group, and the total intraoperative blood loss was less than that in the control group, the difference was statistically significant (P<0.05). After treatment, the ratio of anterior vertebra height in the observation group was higher than that in the control group, and the kyphotic Cobb Angle was lower than that in the control group, with statistical significance (P<0.05). After treatment, the lumbar Dysfunction Index (ODI) score of the observation group was lower than that of the control group, and the Japanese Orthopaedic Association (JOA) score was higher than that of the control group, with statistical significance (P<0.05). Conclusion: In the treatment of thoracolumbar fracture, posterior short segment combined with unilateral fixation of the injured vertebra to the opposite transvertebral arch can improve perioperative indexes, alleviate clinical symptoms, and optimize imaging parameters of the injured vertebra, which is feasible.
