摘  要:目的分析肥胖青少年糖代谢紊乱与骨转化标志物水平的相关性。方法以2022年6月至2023年6月我院收治的50例肥胖青少年为研究对象,以同期接受健康体检的体质量正常的50例青少年为对照组。比较两组一般资料、糖代谢指标以及骨转换标志物水平,采用典型相关性分析肥胖组糖代谢指标和骨转换标志物水平的相关性。将肥胖组根据血糖分为高血糖组和血糖正常组,采用Kendall相关性分析骨转换标志物和高血糖发生风险的相关性。结果肥胖组体质指数(BMI)、空腹血糖(FBG)、胰岛素抵抗指数(HOMA-IR)、胰岛素样生长因子1(IGF-1)、胰岛素样生长因子结合蛋白3(IGFBP-3)、总1型胶原氨基端延肽(P1NP),B-胶原蛋白降解产物(B-CTX)均高于对照组(P<0.05);典型相关性分析结果显示糖代谢指标和骨转换标志物之间存在相关性,第一对(r=0.968)、第二对(r=0.452)典型变量相关性显著(P<0.05),在糖代谢指标中由胰岛素和HOMA-IR决定,在骨转换标志物中由P1NP和B-CTX决定,且典型变量中HOMA-IR分别与P1NP以及B-CTX呈正相关,胰岛素分别与P1NP以及B-CTX呈负相关;高血糖组HOMA-IR、FBG、IGFBP-3、IGF-1、P1NP、B-CTX均高于血糖正常组(P<0.05);Kendall分析显示高血糖发生风险与P1NP(r=0.715)、B-CTX(r=0.530)、N-MID(r=0.566)均具有相关性(P<0.05)。结论肥胖青少年伴有糖代谢紊乱,且糖代谢的紊乱可影响骨转换标志物,以P1NP和B-CTX的改变最为明显。Objective To analyze the correlation between glucose metabolism disorder and bone turnover markers in obese adolescents.Methods From June 2022 to June 2023,50 obese adolescents in our hospital were taken as research group,and 50 adolescents with normal weight who received health examination at the same time were taken as control group.General data,glucose metabolism indexes and bone turnover markers were compared,and the correlation between glucose metabolism indexes and bone turnover markers in obese group was analyzed by canonical correlation.Obesity group was divided into hyperglycemia group and normal blood sugar group according to blood sugar,and the correlation between bone turnover markers and the risk of hyperglycemia was analyzed by Kendall correlation.Results Body mass index(BMI),fasting blood glucose(FBG),insulin resistance index(HOMA-IR),insulin-like growth factor 1(IGF-1),insulin-like growth factor binding protein 3(IGFBP-3),total type 1 collagen amino-terminal peptide(P1NP)and B-collagen degradation product(B-CTX)in obese group were all higher than those in control group(P<0.05).The correlation between the first pair(r=0.968)and the second pair(r=0.452)was significant(P<0.05),which was determined by insulin and HOMA-IR in glucose metabolism indexes,P1NP and B-CTX in bone turnover markers,and HOMA-IR and P1NP and B-CTX in typical variables.HOMA-IR,FBG,IGFBP-3,IGF-1,P1NP and B-CTX in hyperglycemia group were higher than those in normal blood sugar group(P<0.05).Kendall analysis showed that the risk of hyperglycemia was correlated with P1NP(r=0.715),B-CTX(r=0.530)and N-MID(r=0.566)(P<0.05).Conclusion Obese adolescents are accompanied by disorder of glucose metabolism,which could affect bone turnover markers,especially P1NP and B-CTX.
