摘 要:目的采用Meta分析评价卵巢癌组织中YKL-40的表达对卵巢癌的诊断价值。方法检索外文及中文数据库资源中卵巢癌组织中YKL-40的表达诊断卵巢癌的相关文献,检索时限从建库至2022年6月,根据标准筛选文献提取资料。采用Stata 16.0软件分析组织YKL-40诊断卵巢癌的效能。结果最终纳入11篇文献共1962例患者,包含974例卵巢癌和988例非卵巢癌患者。组织YKL-40诊断卵巢癌的敏感度(sensitivity,Sen)为0.75(95%CI 0.70~0.79)、特异度(specificity,Spe)为0.75(95%CI 0.64~0.84)、阳性似然比(positive likelihood ratio,PLR)为3.00(95%CI 2.10~4.40)、阴性似然比(negative likelihood ratio,NLR)为0.33(95%CI 0.27~0.41)、诊断比值比(diagnosis odds ratios,DOR)为9.00(95%CI 5.00~16.00)、综合受试者工作特征曲线(summary receiver operating characteristic curves,SROC)曲线下面积(area under curve,AUC)为0.79(95%CI 0.76~0.83)。结论卵巢癌组织YKL-40表达的检测对诊断卵巢癌具有较高价值。Objective The diagnostic value of ovarian cancer by YKL-40 expression in ovarian cancer tissues was evaluated by Meta-analysis.Methods Relevant literatures on the expression of YKL-40 in ovarian cancer tissue for diagnosis of ovarian cancer were searched in foreign and Chinese databases.The search period was from the establishment of the database to June 2022.According to defined inclusion criteria,the literature was selected and the data were extracted.Stata 16.0 software was used to analyze the diagnostic efficacy of tissue YKL-40 in diagnosing ovarian cancer.Results Eleven papers with 1962 patients,including 974 ovarian cancer and 988 non-ovarian cancer patients,were finally included.The sensitivity(Sen)of tissue YKL-40 for diagnosing ovarian cancer was 0.75(95%CI 0.70-0.79),the specificity(Spe)was 0.75(95%CI 0.64-0.84),the positive likelihood ratio(PLR)was 3.00(95%CI 2.10-4.40),the negative likelihood ratio(NLR)was 0.33(95%CI 0.27-0.41),the diagnosis odds ratios(DOR)of 9.00(95%CI 5.00-16.00),and an summary receiver operating characteristic curves(SROC)area under curve(AUC)was 0.79(95%CI 0.76-0.83).Conclusion The detection of YKL-40 expression in ovarian cancer tissues is of high value for the diagnosis of ovarian cancer.
