摘　　要：目的 探讨联合临床和超声特征的量化评分法在腮腺肿物良恶性风险评估中的价值。方法 回顾性分析405例腮腺肿物的临床资料和超声图像,以病理结果为金标准,通过Logistic回归分析,筛选出预测腮腺肿物恶性风险的独立影响因素作为评分指标,计算每个肿物总分。绘制受试者工作特征曲线,寻找良恶性诊断截断值,评估诊断效能。根据总分进行分级,计算各级别恶性率,建立分级系统。结果 自发或触压痛、形态、边界、后方回声、钙化灶、颈面部淋巴结是腮腺肿物恶性风险的独立影响因素。曲线下面积为0.94,选择总分3分(超声分级5级)作为诊断截断值,其约登指数为0.738。超声分级3~6级的恶性率分别为0、9%、55%、96%。结论 联合临床和超声特征的量化评分法对诊断腮腺肿物的良恶性具有较高效能。Objective Exploring the value of a quantitative scoring method combining clinical and ultrasonographic features in the risk assessment of benign and malignant parotid gland masses.Methods Clinical data and ultrasound images of 405 parotid masses were retrospectively analyzed,and the independent influencing factors predicting the risk of malignancy of parotid masses were screened as scoring indexes by Logistic regression analysis using pathological findings as the gold standard,and the total score for each mass was calculated.The receiver operating characteristic curves of the subjects were plotted to find the benign and malignant diagnostic cutoff values and to assess the diagnostic efficacy.According to the total score,the viciousness rate of each level was calculated,and a grading system was established.Results Spontaneous or palpable tenderness,morphology,borders,posterior echogenicity,foci of calcification,and cervicofacial lymph nodes were independent predictors for malignancy in parotid masses.The area under the curve was O.94,and a total score of 3(ultrasound grade 5)was chosen as the diagnostic cutoff value,with a Youden index of 0.738.The malignancy rates for ultrasound grades 3 to 6 were 0,9%,55%,and 96%,respectively.Conclusions A quantitative scoring method combining clinical and ultrasonographic features is highly effective in diagnosing the benign and malignant nature of parotid gland masses.
