摘　　要：目的探讨主动脉瓣狭窄(AS)患者经导管主动脉瓣置换术(TAVR)后即刻左心室(LV)舒张功能变化。方法研究回顾性纳入接受TAVR治疗的80例AS患者,比较术前和术后即刻LV舒张功能指标变化。结果术后即刻LV舒张功能改善患者30例(37.5%),与术前相比,术后即刻二尖瓣舒张晚期血流速度A峰、二尖瓣环室间隔侧组织舒张早期运动速度e′峰、侧壁侧组织舒张早期运动速度e′峰、等容舒张期增加(P<0.05),而三尖瓣反流峰速2.8 m/s构成比例、左心房容积指数降低(P<0.05)。有创血流动力学参数显示与术前相比,术后即刻左心室舒张末压减低(P<0.05)。结论37.5%患者TAVR术后即刻左心室舒张功能改善。Objective To investigate the changes of left ventricular(LV)diastolic function immediately after transcatheter aortic valve replacement(TAVR).Methods A total of 80 patients undergoing TAVR treatment were retrospectively enrolled in this study.The differences of LV diastolic function paremeters were analyzed before and immediately after TAVR.Results There were 30(37.5%)patients obtaining improvement of LV diastolic function immediately after TAVR.There were improvement in paremeters of diastolic dysfunction grade such as mitral flow velocity early diastolic A peak,septal e′,lateral e′,isovolumic relaxation time,tricuspid regurgitation,and left atrial volume index immediately after TAVR(P<0.05).Invasive hemodymic parameters showed a significant decrease in left ventricular end-diastolic pressure immediately after TAVR compared with pre-TAVR(P<0.05).Conclusions The improved LV diastolic function was experienced in 37.5%of patients immdiately after TAVR.
