产前超声诊断胎儿外生殖器异常的价值

摘　　要：目的探讨胎儿外生殖器异常的声像图特征,提高产前超声对胎儿外生殖器畸形的准确诊断能力。方法回顾性分析产前超声诊断为外生殖器异常胎儿的声像图特征及其随访结果。结果26例外生殖器异常胎儿产后确诊异常24例,超声诊断准确度为92.3%(24/26)。产后发现5种畸形:(1)尿道下裂17例(17/24,70.8%),超声表现:阴茎头端圆钝,向腹侧弯曲,排尿线异常,部分呈“郁金香”征;(2)阴茎阴囊转位不合并尿道下裂2例(2/24,8.3%),超声表现:短小的阴茎位于分裂的阴囊后方,头端指向肛门侧,呈“郁金香”征;(3)隐匿性阴茎2例(2/24,8.3%),超声表现:阴茎体平直,位于阴囊内,显露部短;(4)阴茎短小2例(2/24,8.3%),超声表现:阴茎长度及宽度较同孕龄胎儿小,阴茎头端呈点状稍高回声;(5)阴茎分裂1例(1/24,4.2%),超声表现:会阴部结构杂乱,阴囊分裂较远,阴茎形态失常。结论掌握胎儿外生殖器异常产前声像图特征,对提高其准确诊断能力有重要价值。Objective To investigate the ultrasonographic features of fetal external genitalia abnormalities and improving the diagnostic accuracy rate of prenatal ultrasound.Methods The ultrasound features and follow-up results of 26 fetuses diagnosed with abnormal external genitalia by prenatal ultrasound were retrospectively analyzed.Results Among the 26 cases of abnormal external genitalia,24 cases were diagnosed correctly,with a diagnostic accuracy of 92.3%(24/26).Five types of abnormalities were found in the external genitalia:(1)There were 17 cases of hypospadias(17/24,70.8%).The major ultrasonographic features included blunt penile tip,penile ventral curvature,abnormal urination line,and“tulip”sign of external genitalia;(2)There were 2 cases of penoscrotal transposition without hypospadias(2/24,8.3%).Ultrasound showed that the penis was located behind the divided scrotum and pointed towards the anal side,presenting a“tulip”sign;(3)There were 2 cases of concealed penis(2/24,8.3%).Ultrasound showed a straight penis body located within the scrotum,with a short exposure length;(4)There were 2 cases of micropenis(2/24,8.3%).Ultrasound showed that the length and width of the penis were smaller than those fetuses of the same gestational age,and the tip of the penis showed punctate echoes;(5)One case of penile division(1/24,4.2%).Ultrasound showed disorder in the perineum,distant division of the scrotum,and abnormal morphology of the penis.Conclusions Mastering the ultrasound features of abnormal external genitalia in fetuses is of great value in improving the accuracy of diagnostic ability.
