摘 要:目的采用离体组织和活体动物实验,论证声辐射力脉冲(ARFI)成像方法监测、评估高强度聚焦超声(HIFU)热损伤的可行性。方法本研究基于声辐射力脉冲成像搭建了一个HIFU损伤监测系统。在HIFU消融前、后,用HIFU治疗模块在不同声功率下产生声辐射力激励焦域组织产生运动,并应用超快速超声成像对组织运动进行精准检测。最后,将治疗前后所得二维运动图像进行差分运算,得到可精准反映HIFU治疗后组织损伤状况的差分ARFI影像。结果经HIFU消融后的焦域组织硬度变大,焦域组织运动幅度高出治疗前近10μm;此外,差分ARFI影像中的损伤区域与超声影像中的高回声区域有较高吻合度,且具有更高的损伤对比度和更清晰的损伤边界,表明差分ARFI影像可以更好地反映组织损伤情况。结论离体组织和活体动物实验证明,超声差分ARFI方法可定性判别组织损伤情况,有潜力为临床HIFU热损伤评估提供一种更准确的监测方法。Objective To demonstrate the feasibility of using acoustic radiation force impulse(ARFI)imaging method to monitor and evaluate the thermal damage caused by high intensity focused ultrasound(HIFU)through in vitro tissue and in vivo animal experiments.Methods This study developed a HIFU damage monitoring system based on ARFI imaging.Before and after HIFU ablation,the HIFU treatment module was used to generate acoustic radiation forces at different sound powers to induce motion in the focal tissue,and cutting-edge ultrafast ultrasound imaging was applied to precisely detect tissue motion.Finally,differential calculation was performed on the two-dimensional motion images obtained before and after treatment to generate differential ARFI images,which can accurately reflect the tissue damage condition post-HIFU treatment.Results After HIFU ablation,the hardness of the focal tissue increased,and the movement amplitude of the focal tissue was nearly 10μm higher than before treatment.Additionally,the damaged areas in the differential ARFI images highly matched the high-echo areas in the B-mode ultrasound images,and exhibited higher damage contrast and clearer damage boundaries,indicating that differential ARFI imaging could better reflect tissue damage conditions.Conclusions In vitro tissue and in vivo animal experiments demonstrated that the ultrasound differential ARFI method can qualitatively discern tissue damage,potentially offering a more accurate monitoring method for clinical assessment of HIFU-induced thermal damage.
