摘 要:目的探讨在便携超声引导下构建肝脏穿透伤动物模型的可行性及效果。方法选用巴马猪12只,在便携超声引导下利用微创旋切设备在实验猪肝右叶较厚处进行穿刺致伤,即刻行常规超声及超声造影检查,并记录实验动物的基本生命体征。出血3 h后处死动物并解剖。结果12只实验动物均建模成功,较建模前血压降低,血氧饱和度降低,心率增快(P<0.05),符合失血性休克的表现。建模后超声造影均显示肝脏穿透伤区有造影剂外溢、浓聚,证实存在活动性出血。处死后大体病理证实肝脏穿透伤。结论应用便携超声引导经皮旋切的方法可以建立肝脏穿透伤活动性出血模型,该模型稳定性好,成功率高,为后续肝脏穿透伤的止血治疗提供实验依据。Objective This paper discussed the feasibility and effect of constructing an animal model of liver penetrating trauma under the guidance of portable ultrasound.Methods Twelve Bama pigs were selected.Under the guidance of portable ultrasound,a minimally invasive rotary cutting device was used to puncture the thick part of the right lobe of the experimental pig liver,and then conventional ultrasound and contrast-enhanced ultrasound were performed immediately,and the basic vital signs of the experimental animals were recorded.Three hours after hemorrhage,the animals were sacrificed and dissected.Results All the 12 experimental animals were successfully modeled,and their blood pressure and blood oxygen saturation were lower than those before modeling,and their heart rate was higher than that before modeling(P<0.05),which was consistent with the performance of hemorrhagic shock.After modeling,CEUS showed that there was extravasation and concentration of contrast agent in the penetrating injury area of the liver,which confirmed the existence of active bleeding.Penetrating liver injury was confirmed by gross pathology after death.Conclusions The liver penetrating injury active bleeding model can be established by the method of percutaneous cutting guided by portable ultrasound.The model has good stability and high success rate,which provides experimental basis for the subsequent hemostatic treatment of liver penetrating injury.
