儿童颅面部朗格汉斯细胞组织细胞增生症的多模态超声诊断与治疗评估

摘　　要：目的分析儿童颅面部朗格汉斯细胞组织细胞增生症(LCH)的超声声像图特点,探讨其诊疗价值。方法回顾性分析两院收治的10例经病理确诊为LCH患儿的临床及超声资料,总结分析其声像图特征。结果(1)10例患儿均以发现颅面部肿物为主诉,颅面部共16处病灶。(2)7例患儿行超声实时引导下颅面部肿物穿刺活检术,3例行病损切除术。(3)超声表现:a.10例均出现溶骨性骨破坏,局部骨皮质毛糙、连续性中断。b.10例患儿骨缺损区均见低回声填充。c.病灶血供丰富程度不一。(4)随访结果:7例行超声引导下穿刺活检术患儿复查骨质破坏均较前修复、软组织肿块缩小。结论超声在颅面部朗格汉斯细胞组织细胞增生症中具有独特声像图特征,同时可引导病灶穿刺活检术,在优化诊疗方案、治疗随访中均具有较高价值。Objective To analyze the sonographic characteristics of Langerhans cell histiocytosis(LCH)in children's craniofacial region and to explore its diagnostic and therapeutic value.Methods The clinical and ultrasound data of 10 cases of children with LCH diagnosed pathologically were retrospectively analyzed,and their sonographic characteristics were summarized and analyzed.Results(1)The chief complaint of 10 cases was craniofacial tumor,and a total of 16 lesions were found in the craniofacial region.(2)7 cases underwent ultrasound-guided biopsy of craniofacial tumors,and 3 underwent lesion resection.(3)Ultrasound findings:a.Osteolytic bone destruction occurred in 10 cases,and the local bone cortex was rough and the continuity was interrupted.b.Hypoechoic filling was found in the bone defect area in 10 cases.c.The lesion had different blood supply richness.(4)Follow up results:The bone destruction of 7 cases of ultrasound-guided puncture biopsy was smaller than that of the previous repair and soft tissue mass.Conclusions Ultrasound has unique sonographic characteristics in craniofacial Langerhans cell histiocytosis,and can guide the puncture biopsy of the focus,which is valuable for the optimization of diagnosis and treatment plan and follow-up post treatment.
