超声实时引导下胸锁关节炎的介入治疗研究

摘　　要：目的胸锁关节炎是一种罕见且通常伴有疼痛和局部隆起的疾病。本研究旨在评估超声引导下药物注射治疗胸锁关节炎的有效性。方法本研究纳入了22例胸锁关节炎患者[男性18例,女性4例,平均年龄(42.75±11.19)岁],均接受了超声引导下的胸锁关节间隙药物注射。通过门诊及电话随访,记录治疗前后患者的胸锁关节疼痛视觉模拟评分(VAS)及胸锁关节间隙宽度(L),并进行比较。结果治疗前所有患者均有胸锁关节区疼痛和体表隆起症状,7例患者上肢抬举受限。初步筛查显示,8例C-反应蛋白(CRP)阳性,2例红细胞沉降率(ESR)增高,2例人类白细胞抗原B27(HLA-B27)阳性,6例抗核抗体(ANA)阳性,4例尿酸增高。术后关节间隙显著缩小,血流信号减少。治疗前后对比显示,术后1周和1个月的关节间隙宽度和VAS评分均显著降低(P<0.01),表明疼痛显著缓解,治疗有效。结论超声引导下的关节间隙药物注射是一种对胸锁关节炎患者有效、简便且安全的治疗方法。Objective Sternoclavicular arthritis is a rare condition typically accompanied by pain and localized swelling.This study aims to evaluate the efficacy of ultrasound-guided medication injections in treating sternoclavicular arthritis.Methods The study included 22 patients with sternoclavicular arthritis[18 males and 4 females,average age(42.75±11.19)years],all of whom underwent ultrasound-guided injections into the sternoclavicular joint space.Outcomes were assessed through clinic visits and telephone follow-ups,documenting patients'pain levels using the visual analog scale(VAS)and the width of the sternoclavicular joint space(L)before and after treatment.Results All patients presented with pain and swelling in the sternoclavicular area,and 7 had restricted arm elevation.Preliminary screening showed 8 cases of positive C-reactive protein(CRP),2 cases of elevated erythrocyte sedimentation rate(ESR),2 cases of positive human leukocyte antigen B27(HLA-B27),6 cases of positive antinuclear antibodies(ANA),and 4 cases of elevated uric acid.Post-treatment assessments revealed significant reductions in joint space and decreased blood flow signals.Comparative analyses before and after treatment indicated significant reductions in both joint space width and VAS scores at one week and one month post-treatment(P<0.01),demonstrating significant pain relief and effective treatment.Conclusions Ultrasound-guided injections into the joint space provide an effective,convenient,and safe treatment option for patients with sternoclavicular arthritis.
