摘 要:目的分析血清性激素结合球蛋白(SHBG)、25羟维生素D3[25(OH)D3]、Ⅰ型前胶原氨基端延长肽(P1NP)在特发性中枢性性早熟(ICPP)女童中的表达及相关性,以期为临床尽早诊断及治疗ICPP女童提供依据。方法选取2018年11月-2022年11月内蒙古自治区妇幼保健院儿内科收治的ICPP女童122例(观察组)作为研究对象,选取同期在本院体检的同龄正常发育女童57名作为对照组。收集所有受试者一般临床资料,检测并比较两组血清SHBG、25(OH)D3、P1NP、促黄体生成素(LH)、促卵泡生成素(FSH)及雌二醇(E_(2))水平,采用Pearson模型分析SHBG、25(OH)D3、P1NP与ICPP女童性激素指标水平的相关性,构建受试者工作特征(ROC)曲线分析SHBG、25(OH)D3及P1NP对ICPP的预测价值。结果观察组SHBG、25(OH)D3水平低于对照组,而P1NP、LH、FSH、E_(2)水平高于对照组,差异均有统计学意义(t=8.961、9.931、11.018、15.999、12.693、13.944,P均<0.05)。Pearson模型分析显示,SHBG及25(OH)D3与LH、FSH及E_(2)水平均成负相关(r=-0.527、-0.534、-0.437,-0.623、-0.419、-0.324,P均<0.05),P1NP与LH、FSH及E_(2)水平均成正相关(r=0.319、0.631、0.587,P均<0.05)。SHBG、25(OH)D3及P1NP三者联合检测预测ICPP的曲线下面积(AUC)为0.970,高于SHBG、25(OH)D3及P1NP单独检测的0.845、0.861、0.910,差异均有统计学意义(P均<0.05)。结论ICPP女童血清SHBG、25(OH)D3水平异常下降,P1NP水平异常上升,均与性激素相关,联合检测血清SHBG、25(OH)D3及P1NP水平对预测ICPP的价值较高,有助于临床更好地诊断及治疗ICPP。Objective To analyze the expression and correlation of serum sex hormone binding globulin(SHBG),25⁃hydroxyvitamin D3[25(OH)D3],and type I procollagen amino terminal extension peptide(P1NP)in girls with idiopathic central precocious puberty(ICPP),and provide evidence for early diagnosis and treatment of ICPP in girls.Methods A total of 122 ICPP girls(observation group)admitted to the Department of Pediatrics,Maternal and Child Health Hospital of Inner Mongolia Autonomous Region from November 2018 to November 2022 were selected as the research objects,and 57 normal development girls of the same age who underwent physical examination in our hospital during the same period were selected as the control group.The general clinical data of all subjects were collected.The levels of serum SHBG,25(OH)D3,P1NP,luteinizing hormone(LH),follicle stimulating hormone(FSH)and estradiol(E_(2))were detected and compared between the two groups.Pearson model was used to analyze the correlation between SHBG,25(OH)D3,P1NP and sex hormone levels in ICPP girls.The receiver operating characteristic(ROC)curve was constructed to analyze the predictive value of SHBG,25(OH)D3 and P1NP for ICPP.Results The levels of SHBG and 25(OH)D3 in the observation group were lower than those in the control group,while the levels of P1NP,LH,FSH and E_(2)were higher than those in the control group,and the differences were statistically significant(t=8.961,9.931,11.018,15.999,12.693,13.944;all P<0.05).Pearson model analysis showed that SHBG and 25(OH)D3 were negatively correlated with LH,FSH and E_(2)levels(r=-0.527,-0.534,-0.437;-0.623,-0.419,-0.324;all P<0.05),and P1NP was positively correlated with LH,FSH and E_(2)levels(r=0.319,0.631,0.587;all P<0.05).The area under curve(AUC)of ICPP predicted by combined detection of SHBG,25(OH)D3 and P1NP was 0.970,which was significantly higher than that of SHBG,25(OH)D3 and P1NP alone(0.845,0.861,0.910)(all P<0.05).Conclusions The levels of serum SHBG and 25(OH)D3 in girls with ICPP decreased abnormally,and the level
