摘 要:目的分析妊娠早中期孕妇血清叉头蛋白O1(FOXO1)、胎盘生长因子(PLGF)水平对早发型子痫前期的预测价值,以期为预测妊娠早中期孕妇发生早发型子痫前期提供一定的依据。方法选取2021年1月-2022年6月在北京市大兴区人民医院确诊的180例发生早发型子痫前期的妊娠早中期孕妇作为观察组,及同期孕检的185名正常的妊娠早中期孕妇作为对照组。采用酶联免疫吸附(ELISA)法检测妊娠早中期孕妇血清FOXO1、PLGF水平;Pearson分析早发型子痫前期孕妇血清FOXO1与PLGF水平的相关性;Logistic回归分析影响妊娠早中期孕妇发生早发型子痫前期的因素;受试者工作特征(ROC)曲线分析血清FOXO1、PLGF水平对妊娠早中期孕妇发生早发型子痫前期的预测价值。结果观察组早发型子痫前期孕妇血清FOXO1、PLGF水平均显著低于对照组,差异均有统计学意义(t=18.189、17.856,P均<0.05)。早发型子痫前期孕妇血清FOXO1与PLGF水平成正相关(r=0.410,P<0.05)。Logistic回归分析发现,FOXO1、PLGF是妊娠早中期孕妇发生早发型子痫前期的保护因素(P均<0.05)。血清FOXO1、PLGF联合检测预测妊娠早中期孕妇发生早发型子痫前期的曲线下面积(AUC)为0.953,优于FOXO1、PLGF单独检测的0.895、0.916,差异均有统计学意义(Z=3.074、2.151,P均<0.05)。结论早发型子痫前期孕妇血清FOXO1、PLGF水平降低,对妊娠早中期孕妇发生早发型子痫前期具有一定的预测价值。Objective To analyze the predictive value of forkhead box protein O1(FOXO1)and placental growth factor(PLGF)levels in serum for early onset preeclampsia in pregnant women in early and middle pregnancy.Methods A total of 180 pregnant women in early and middle pregnancy with early onset preeclampsia diagnosed in the Beijing Daxing District People's Hospital from January 2021 to June 2022 were selected as the observation group,and 185 normal pregnant women in early and middle pregnancy at the same time as the control group.Enzyme linked immunosorbent assay(ELISA)was used to detect the levels of FOXO1 and PLGF in the serum of pregnant women in early and middle pregnancy;the correlation between FOXO1 and PLGF levels in serum of pregnant women with early onset preeclampsia was analyzed by Pearson.Logistic regression analysis was used to analyze the factors influencing the occurrence of early onset preeclampsia in early and middle pregnancy;the predictive value of serum FOXO1 and PLGF levels for the occurrence of early onset preeclampsia in early and middle pregnancy was analyzed by receiver operating characteristic(ROC)curve.Results The levels of serum FOXO1 and PLGF in the observation group were significantly lower than the control group(t=18.189,17.856;both P<0.05).There was a positive correlation between serum FOXO1 and PLGF levels in early onset preeclampsia(r=0.410,P<0.05).Logistic regression analysis showed that FOXO1 and PLGF were protective factors for early onset preeclampsia in early and middle pregnancy(both P<0.05).The area under the curve(AUC)of serum FOXO1 combined with PLGF for early onset preeclampsia in early and middle pregnancy was 0.953,which was better than FOXO1 and PLGF alone(0.895,0.916);the differences were statistically significant(Z=3.074,2.151;both P<0.05).Conclusion The levels of serum FOXO1 and PLGF in pregnant women with early onset preeclampsia decreased,which had certain predictive value for early onset preeclampsia in early and middle pregnancy.
