摘　　要：细胞衰老是一种与机体衰老相关的不可逆的细胞周期停滞状态。慢性阻塞性肺疾病(COPD)作为典型的衰老相关性肺部疾病,组织及器官中衰老细胞的增加是衰老的重要组成部分,年龄是其发展的最重要危险因素之一。年龄相关的端粒缩短是复制性衰老的主要诱因,但氧化应激、代谢功能紊乱和表观遗传的改变等因素也可促使机体进入衰老状态。香烟烟雾(CS)诱导线粒体损伤伴线粒体活性氧(mtROS)产生增加,可导致线粒体形态和自噬功能异常,加速COPD细胞衰老。因此,靶向细胞衰老疗法及如何利用中医“气血理论”干预细胞衰老,已然成为COPD的研究热点。中医学认为“肺脏虚损,痰瘀互结”是导致COPD细胞衰老的病理本质。因而从线粒体自噬角度探讨细胞衰老与COPD的相关性,挖掘中医药防治COPD的潜在新靶点,具有重大的临床意义和广泛的应用价值。Cell senescence is an irreversible cell cycle arrest state associated with aging.Chronic obstructive pulmonary disease(COPD)is a typical aging-related lung disease.The increase of senescent cells in tissues and organs is an important part of aging,and age is one of the most important risk factors for its development.Age-related telomere shortening is the main inducement of replicative senescence,but factors of oxidative stress,metabolic dysfunction and epigenetic changes can also prompt body to enter the senescent state.Cigarette smoke(CS)induces mitochondrial damage with increased production of mitochondrial reactive oxygen species(mtROS),leading to abnormal mitochondrial morphology and autophagy function,which accelerates cell senescence in COPD.Therefore,targeted cell senenscence therapy and how to use the"Qi and blood theory"of traditional Chinese medicine to intervene in cell senenscence have become the research hotspots of COPD.Traditional Chinese medicine believes that"lung deficiency,phlegm and stasis obstructing the internal"is the pathological nature of cell senenscence in COPD.This paper discusses the correlation between cell senenscence and COPD from the perspective of mitophagy,and explores potential new targets of traditional Chinese medicine in the prevention and treatment of COPD,which has great clinical significance and wide application value.
