摘　　要：目的比较腹泻型肠易激综合征湿热质与非湿热质患者外周血长链非编码RNA X染色体失活特异转录因子(lncRNA XIST)、长链非编码RNA牛磺酸上调基因1(lncRNA TUG1)表达的差异并分析其临床意义。方法选择2021年1月至2023年12月期间江苏省如皋市中医院收治的120例腹泻型肠易激综合征患者作为观察组,根据中医体质分为湿热质亚组和非湿热质亚组;选择同期体检的100名健康者作为对照组。检测外周血lncRNA XIST、lncRNA TUG1表达水平及血清C反应蛋白(CRP)水平并比较差异,分析腹泻型肠易激综合征湿热质的影响因素及诊断指标。结果观察组患者外周血lncRNA XIST的表达水平高于对照组,lncRNA TUG1的表达水平低于对照组,差异有统计学意义(t=8.136、9.324,P<0.05);观察组中湿热质亚组患者外周血lncRNA XIST的表达水平及血清CRP的水平高于非湿热质亚组,lncRNA TUG1的表达水平低于非湿热质亚组,差异有统计学意义(P<0.05);lncRNA XIST是腹泻型肠易激综合征湿热质的危险因素,lncRNA TUG1是腹泻型肠易激综合征湿热质的保护因素(P<0.05);外周血lncRNA XIST、lncRNA TUG1诊断腹泻型肠易激综合征湿热质的ROC曲线下面积分别为0.754和0.810。结论lncRNA XIST表达增加、lncRNA TUG1表达降低与湿热质腹泻型肠易激综合征相关,外周血中两项指标的表达水平对湿热质腹泻型肠易激综合征具有诊断价值。Objective To compare the differences of lncRNA X inactive specific transcription factor(lncRNA XIST)and lncRNA taurine upregulation gene 1(lncRNA TUG1)expression in peripheral blood between dampness-heat and non-dampness-heat diarrheal irritable bowel syndrome patient and analyze its clinical significance.Methods 120 cases of diarrheal irritable bowel syndrome treated at Rugao Hospital of Traditional Chinese Medicine in Jiangsu Province from January 2021 to December 2023 were selected as the observation group.They were divided into two subgroups:dampness-heat subgroup and non-dampness-heat subgroup based on TCM constitution.100 healthy subjects who underwent physical examination during the same period were selected as the control group.The expression levels of lncRNA XIST and lncRNA TUG1 in peripheral blood and the level of C-reactive protein(CRP)in serum were detected and compared.The influencing factors and diagnostic indicators of dampness-heat diarrheal irritable bowel syndrome were analyzed.Results The expression level of lncRNA XIST in the peripheral blood of the observation group was higher than that of the control group,and the expression level of lncRNA TUG1 was lower than that of the control group,with statistical significance(t=8.136,9.324,P<0.05).Within the observation group,the expression level of lncRNA XIST in peripheral blood and the CRP level in serum of the dampness-heat subgroup were higher than those in the non-dampnessheat subgroup,and the expression level of lncRNA TUG1 was lower than that in the non-dampness-heat subgroup,with statistical significance(P<0.05).LncRNA XIST was identified as a risk factor for dampness-heat diarrheal irritable bowel syndrome,and lncRNA TUG1 was found to be a protective factor for dampness-heat diarrheal irritable bowel syndrome(P<0.05).The areas under the ROC curves of lncRNA XIST and lncRNA TUG1 in peripheral blood for the diagnosis of dampness-heat diarrheal irritable bowel syndrome were 0.754(P<0.05)and 0.810(P<0.05),respectively.Conclusion The increased exp
