血清H2、E_(2)与25(OH)D水平对围绝经期女性盆底功能及阴道脱落细胞成熟度的影响

摘  要:目的分析松弛素(H2)、雌激素(E_(2))与25-羟维生素D[25(OH)D]水平对围绝经期女性盆底功能及阴道脱落细胞成熟度的影响。方法选取2022年5月至2024年6月于廊坊市人民医院就诊的围绝经期盆底功能障碍性疾病(PFD)女性79例为观察组,另选取同期于本院进行体检的80名健康围绝经期女性(未合并盆腔疾病)为对照组。对比两组血清H2、E_(2)、25(OH)D水平、盆底功能以及阴道脱落细胞成熟度;采用二元logistic多因素分析影响围绝经期患者盆底功能及阴道脱落细胞成熟度的相关因素;采用皮尔斯相关系数分析H2、E_(2)、25(OH)D水平与围绝经期患者盆底功能及阴道脱落细胞成熟度的相关性。结果观察组H2水平高于对照组,E_(2)、25(OH)D水平低于对照组,差异有统计学意义(P<0.05);观察组静息肌电值大于对照组,最大收缩力、持续收缩力小于对照组,差异有统计学意义(P<0.05);观察组阴道脱落细胞成熟度低于对照组,差异有统计学意义(P<0.05);经过二元logistic多因素分析显示,H2水平升高以及E_(2)、25(OH)D水平降低是影响围绝经期患者盆底功能、阴道脱落细胞成熟度的独立危险因素(P<0.05);根据Pearson相关性分析可得:H2与静息肌电值、最大收缩力、持续收缩力、阴道脱落细胞成熟度呈负相关;E_(2)、25(OH)D与静息肌电值、最大收缩力、持续收缩力、阴道脱落细胞成熟度呈正相关(P<0.05)。结论围绝经期PDF患者血清H2水平升高,E_(2)、25(OH)D水平降低,三指标均与盆底功能、阴道脱落细胞成熟度存在一定关联。Objective To analyze the effects of serum relaxin(H2),estrogen(E_(2))and 25-hydroxy vitamin D[25(OH)D]levels on pelvic floor function and vaginal exfoliated cell maturity in perimenopausal women.Methods A total of 79 perimenopausal women with pelvic floor dysfunction(PFD)who were treated in our hospital from May 2022 to June 2024 were selected as the observation group,and 80 healthy perimenopausal women(without pelvic diseases)who underwent physical examinations in our hospital during the same period were selected as the control group.The levels of H2,E_(2),25(OH)D,pelvic floor function and vaginal exfoliated cell maturity were compared between the two groups.The Pearson correlation coefficient was used to analyze the correlation between H2,E_(2),25(OH)D levels and pelvic floor function as well as vaginal exfoliated cell maturity in perimenopausal patients.Results The levels of H2 in the observation group were higher than those in the control group,and the levels of E_(2)and 25(OH)D were lower than those in the control group,the differences were statistically significant(P<0.05).The resting electromyography value of the observation group was greater than that of the control group,and the maximum contraction force and continuous contraction force were smaller than those in the control group,the differences were statistically significant(P<0.05).The maturity of vaginal exfoliated cells in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).According to Pearson correlation analysis,H2 was negatively correlated with resting myoelectric value,maximum contractility,sustained contractility and maturity of vaginal exfoliated cells.Binary logistic multivariate analysis showed that H2,E_(2)and 25(OH)D levels were independent risk factors affecting pelvic floor function and maturity of vaginal exfoliated cells in perimenopausal patients(P<0.05).E_(2)and 25(OH)D were positively correlated with resting myoelectric value,maximum contractility,sustained contractili
