参杞强精煎治疗少弱精子症的临床疗效及对体外受精结局的影响

摘　　要：目的探讨参杞强精煎治疗少弱精子症患者的临床疗效及对体外受精(IVF)结局的影响。方法招募2020年10月至2022年12月广西壮族自治区人民医院和广西中医药大学第一附属医院收治的少弱精子症患者209例,根据患者治疗意愿分为观察组(接受参杞强精煎治疗,108例)和对照组(接受左卡尼汀治疗,101例)。治疗3个月后均行IVF助孕,比较两组精液质量及IVF结局。结果两组治疗后精子DNA碎片指数(DFI)均显著降低(P<0.05),且观察组较对照组更低,差异有统计学意义(P<0.05)。对照组精子顶体完整率治疗前后差异无统计学意义(P>0.05),观察组治疗后精子顶体完整率较治疗前显著升高(P<0.05),且较对照组更高,差异有统计学意义(P<0.05)。两组治疗后前向运动精子百分率、精子浓度、精子总数、精子正常形态率比较差异无统计学意义(P>0.05)。观察组受精率、优胚率显著高于对照组(P<0.05)。两组卵裂率、临床妊娠率比较差异无统计学意义(P>0.05)。结论参杞强精煎可提高少弱精子症患者精液质量,优化IVF结局,且对部分指标的改善较左卡尼汀更优。Objective To explore the clinical efficacy of Shenqi Qiangjing Decoction in treating oligoasthenozoospermia patients and its effect on in vitro fertilization(IVF)outcomes.Methods A total of 209 patients with oligoasthenozoospermia who were admitted to the People′s Hospital of Guangxi Zhuang Autonomous Region and the First Affiliated Hospital of Guangxi University of Chinese Medicine from October 2020 to December 2022 were recruited.According to the patients′treatment intentions,they were divided into observation group(receiving Shenqi Qiangjing Decoction treatment,108 cases)and control group(receiving L-carnitine treatment,101 cases).After 3 months of treatment,IVF-assisted pregnancy was performed on all the patients.The semen quality and IVF outcomes were compared between the two groups.Results After treatment,sperm DNA fragmentation index(DFI)was significantly decreased in both groups(P<0.05),and the sperm DFI in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).There was no statistical significance in sperm acrosome integrity rate in the control group before and after treatment(P>0.05),and sperm acrosome integrity rate in the observation group after treatment was significantly higher than that before treatment(P<0.05),and the observation group had higher sperm acrosome integrity rate than the control group,and the differences were statistically significant(P<0.05).There were no significant differences in the sperm progressive motility rate,sperm concentration,total sperm count and normal sperm morphology rate between the two groups after treatment(P>0.05).The fertilization rate and excellent embryo rate in the observation group were significantly higher than those in the control group(P<0.05),and there were no significant differences in cleavage rate and clinical pregnancy rate between the two groups(P>0.05).Conclusion Shenqi Qiangjing Decoction can improve the semen quality of the patients with oligoasthenozoospermia and optimize the IV
