摘  要:目的分析ω-3多不饱和脂肪酸(polyunsaturated fatty acid,PUFA)在<32周且出生体质量<1500g的早产儿治疗中的临床效果及对免疫功能及细胞因子的影响。方法回顾性分析宁波市妇女儿童医院新生儿重症监护室2018年1月至2023年1月收治的出生胎龄<32周且出生体质量<1500g的早产儿临床资料,根据治疗用的脂肪酸类型不同分为ω-3组(n=80)与ω-6组(n=80)。ω-6组早产儿实施ω-6PUFA营养支持,ω-3组早产儿实施ω-3PUFA营养支持。对比两组早产儿的疗效、并发症发生率、治疗前后免疫功能及细胞因子水平。结果ω-3组早产儿出生后第7天C反应蛋白、乳酸(lactic acid,Lac)、总胆汁酸、白细胞介素-8(interleukin,IL-8)与出生后第14天Lac、IL-8、肿瘤坏死因子-α水平、视网膜病变、坏死性小肠结肠炎并发症发生率较ω-6组低,用氧时间短,CD4/CD8水平较ω-6组高。结论ω-3PUFA在早产儿营养支持中疗效显著,可有效调节早产儿的免疫功能,抑制炎性因子释放,且并发症少,安全性高。Objective To analyze the clinical efficacy and its effect on immune function and cytokines ofω-3 polyunsaturated fatty acid(PUFA)in the treatment of preterm infants<32 weeks and birth weight<1500g.Methods The clinical data of preterm infants with birth gestational age<32 weeks and birth weight<1500g admitted to the neonatal intensive care unit of Ningbo Women and Children’s Hospital from January 2018 to January 2023 were retrospectively analyzed,and divided intoω-3 group(n=80)andω-6 group(n=80)according to the type of fatty acid used for treatment.ω-6 group of preterm infants were given nutritional support withω-6PUFA,whileω-3 group of preterm infants were given nutritional support withω-3PUFA.The efficacy,complication rate,immune function and cytokine level before and after treatment were compared between two groups.Results On the 7th day after birth,the levels of C-reactive protein,lactic acid(Lac),total bile acids,interleukin-8(IL-8)and on the 14th day after birth,Lac,IL-8,tumor necrosis factor-α,retinopathy,and necrotizing enterocolitis complications inω-3 group were lower than those inω-6 group and the use of oxygen was shorter;The CD4/CD8 levels were higher in theω-6 group.Conclusionω-3PUFA have remarkable efficacy in nutritional support of preterm infants,which can effectively regulate immune function and inhibit the release of inflammatory factors,with fewer complications and higher safety.
